摘  要:目的探讨老年抑郁症患者血清神经活性甾体水平变化及与认知功能的相关性。方法选取2017年1月~2018年7月期间我院诊治的60例老年抑郁症患者为研究组,选取同时期年龄、文化程度匹配的健康志愿者60例为对照组。检测其血清神经活性甾体相关指标及认知功能,比较两组差异,并进行相关性分析。结果(1)与对照组相比,研究组患者血清DHEA-S、T、E2水平均明显降低(P<0.01)。DSF、DSB、MMSE明显降低(P<0.01),TMTA、TMTB、TMT(B-A)明显增加(P<0.01)。(2)血清DHEA-S、T水平与DSF、DSB、MMSE评分呈正相关(均P<0.05),与TMTA、TMTB、TMT(B-A)值呈负相关(P均<0.05)。血清E2水平与MMSE值呈正相关(P<0.05),与TMTB、TMT(B-A)值呈负相关性(P<0.05),与DSF、DSB、TMTA均无明显相关性(P均>0.05)。血清P水平与DSF、DSB、TMTA、TMTB、TMT(B-A)值、MMSE评分均无明显相关性(P均>0.05)。结论老年抑郁症患者存在血清神经活性甾体DHEA-S、T、E2水平降低及认知功能损害,患者血清DHEA-S、T、E2水平降低与认知功能损害相关。Objective To explore the changes of serum neuroactive steroid levels and their correlation with cognitive function in elderly patients with depression.Methods Sixty elderly patients with depression diagnosed and treated in our hospital from January 2017 to July 2018 were selected as the study group,and 60 healthy volunteers with the same age and education level were selected as the control group.The serum neuroactive steroid-related indicators and cognitive function were detected and compared,and correlation analysis was performed.Results(1)Compared with the control group,serum dehydroepiandrosterone sulfate(DHEA-S),testosterone(T),estradiol(E2),digit span-forward task(DSF),digit span-backward task(DSB),and mini-mental state examination(MMSE)levels in the study group decreased significantly(P<0.01),and trail-making test part A(TMTA),trail-making test part B(TMTB),TMT(B-A)increased significantly(P<0.01).(2)Serum DHEA-S and T levels were positively correlated with DSF,DSB,and MMSE scores(all P<0.05),and negatively correlated with TMTA,TMTB,and TMT(B-A)values(all P<0.05).Serum E2 level was positively correlated with MMSE values(P<0.05),negatively correlated with TMTB and TMT(B-A)values(P<0.05),and had no significant correlation with DSF,DSB,and TMTA(all P>0.05).There was no significant correlation between serum P levels and DSF,DSB,TMTA,TMTB,TMT(B-A)values,and MMSE scores(all P>0.05).Conclusion Decreased serum neuroactive steroids DHEA-S,T,E2 and cognitive impairment are presented in elderly patients with depression,and decreased serum DHEA-S,T,E2 levels are associated with cognitive impairment.
