七氟烷联合右美托咪定在妇科腹腔镜手术中的应用效果

摘　　要：目的观察七氟烷联合右美托咪定在妇科腹腔镜手术的麻醉效果。方法选择2018年1月至2019年12月在本院行腹腔镜妇科手术的患者60例(卵巢囊肿及卵巢良性肿瘤),年龄20~58岁,美国麻醉医师协会(ASA)Ⅰ—Ⅱ级,采用随机数字表法平均分为两组:七氟烷组(S组),七氟烷联合右美托咪定组(SD组)。比较两组患者的麻醉开始前15 min、手术开始40 min及手术结束10 min不同时间段的平均动脉压(MAP)、血氧饱和度(SpO2)、心率(HR)变化,记录术后疼痛评分以及术后并发症。结果两组患者麻醉开始前15 min的MAP、SpO2、HR数值比较,差异无统计学意义(P>0.05)。SD组手术开始40 min及手术结束10 min的MAP、SpO2、HR水平均优于S组,差异有统计学意义(P<0.05)。SD组患者术后2 h、12 h、24 h的疼痛评分低于S组(P<0.05);SD组患者术后并发症的发生率低于S组(P<0.05)。结论七氟烷联合右美托咪用于妇科腹腔镜手术中效果显著,可有效减少术后疼痛和降低不良反应发生率。Objective To observe the anesthetic effect of sevoflurane combined with dexmedetomidine in gynecological laparoscopy.Methods Select line of 60 cases of laparoscopic gynecologic surgery patients,age 20-58,ASAⅠ-Ⅱlevel,were randomly divided into two groups by random number table method(n=30):Sevoflurane group(group S),Sevoflurane combined with Dexmedetomidine group(group SD).The mean arterial pressure(MAP),blood oxygen saturation(SpO2)and heart rate(HR)of the two groups were observed and compared 15 min before the start of anesthesia,40 min after surgery and 10 min after surgery,and the postoperative pain score and postoperative complications were recorded.Results MAP,SpO2 and HR values were compared between the two groups 15 min before the start of anesthesia,and the difference was not statistically significant(P>0.05).MAP,SpO2 and HR in group SD were significantly higher than those in Group S(P<0.05).The Vas scores of 2 h,12 h and 24 h after operation in group SD were lower than those in group S(P<0.05),and the incidence of postoperative complications in group SD was lower than that in group S(P<0.05).Conclusion Perioperative sevoflurane combined with dexmedetomidine can maintain hemodynamic stability,effectively reduce postoperative pain and reduce the incidence of adverse reactions.
