提出一种成人股骨头坏死的早期X线征象

摘　　要：目的:通过分析早期成人股骨头坏死的影像表现(MRI、X线平片),提出一种X线征象(唇样征),便于基层医务工作者,特别是无MRI设备的医疗机构的医务工作者理解掌握X线早期表现,从而提高诊断符合率,减少漏诊。方法:回顾分析我院2013年3月至2019年3月间经MRI与X线确诊的股骨头坏死病例,根据ARCO国际股骨头坏死分期标准,找出符合Ⅱ期诊断的53例患者,56个髋关节,由5位高年资医师复习阅读X线平片,记录发现的X线征象。结果:股骨头前上部局部骨密度略低6例,局部骨密度略高19例,局部骨小梁增粗3例,股骨头中下部弧形硬化带28例,弧形硬化带根据其形象,我们称之为"唇样征"。结论:"唇样征"形象直观、典型,易于掌握理解,是股骨头坏死早期X线主要征象之一。Objective By analyzing the imaging manifestations of early adult femoral head necrosis (MRI,X-ray plain film),an X-ray sign (lip-like sign) was proposed to convenient for grass-roots medical workers to grasp the early X-ray manifestations of femoral head necrosis,especially those in medical institutions without MRI equipment,so as to improve the diagnostic coincidence rate and reduce missed diagnosis.Methods The cases of femoral head necrosis diagnosed by MRI and X-ray in our hospital from March 2013 to March 2019 were retrospective-ly analyzed.According to ARCO international staging standard of femoral head necrosis,53 patients and 56 hip joints were found to measure up the stage Ⅱ diagnosis.Five senior doctors reviewed and read X-ray plain films and recorded the X-ray signs.Results Local bone mineral density of anterior and upper femoral head was slightly lower in 6 cases,slightly higher in 19 cases,local trabecular thickening in 3 cases,and arc-shaped sclerotic zone of middle and lower femoral head in 28 cases.According to its image,arc-shaped sclerotic zone is called "lip-like sign".Conclusion "Lip like sign" is typical and easy to understand.It is one of the main signs of X-ray in early stage of avascular necrosis of the femoral head.
