热敏灸联合清热调血汤对湿热蕴结型痛经患者疗效、镇痛效应及子宫动脉血流影响

摘　　要：目的观察热敏灸联合清热调血汤对湿热蕴结型痛经患者疗效、镇痛效应及子宫动脉血流影响。方法将120例湿热蕴结型痛经患者(病例来源:2018年1月—9月期间秦皇岛市中医医院以及秦皇岛市妇幼保健院中医妇科门诊)随机数字法分为芬必得组(共60例,予以西药芬必得治疗)和热敏调血组(共60例,予以热敏灸+清热调血汤治疗),对比分析热敏调血组和芬必得组的近期(随访3个月)和远期(随访6个月)临床疗效、痛经时VAS评分、炎性相关因子、子宫动脉血流指标等的差异。结果热敏调血组和芬必得组的近期(随访3个月)治疗优良率比较无统计学差异(66.67%vs 63.33%,P>0.05),但热敏调血组的远期(随访6个月)治疗优良率高于芬必得组(88.33%vs 68.33%,P<0.05)。热敏调血组和芬必得组近期(随访3个月)和远期(随访6个月)的痛经时VAS评分、炎性因子(IL-6、TNF-α、hs-CRP)、血流指标(RI)均低于治疗前(P<0.05),且同一随访时刻点比较,热敏调血组的痛经时VAS评分、炎性因子(IL-6、TNF-α、hs-CRP)、血流指标(RI)均低于芬必得组(P<0.05)。热敏调血组和芬必得组近期(随访3个月)和远期(随访6个月)的血流指标(EDV、PSV、PI)均高于治疗前(P<0.05),且同一随访时刻点比较,热敏调血组的痛经时VAS评分、炎性因子血流指标(EDV、PSV、PI)均高于芬必得组(P<0.05)。结论热敏灸联合清热调血汤可显著提升湿热蕴结型痛经治疗的临床远期疗效,可降低机体炎性反映,改善患者痛经症状和子宫动脉血流指标。Objective To observe the effect of heat-sensitive moxibustion combined with Qingre Tiaoxue Decoction on dysmenorrhea of damp-heat accumulation type,analgesic effect and uterine artery blood flow.Methods 120 cases of damp-heat accumulation dysmenorrhea were randomly divided into Fenbid group(60 cases were treated with fenbid)and heat-sensitive blood-regulating group(60 cases were treated with Fenbid and heat-sensitive moxibustion plus Qingre Tiaoxue Decoction).The short-term(follow-up for 3 months)and long-term(follow-up for 6 months)clinical efficacy and VA in dysmenorrhea were compared between heat-sensitive blood-regulating group and Fenbid group.The differences of VAS score,inflammatory related factors and uterine artery blood flow index were observed.Results There was no difference in the short-term(follow-up for 3 months)excellent and good rate between the heat-sensitive blood-regulating group and the Fenbid group(66.67%vs 63.33%,P>0.05),but the long-term(follow-up for 6 months)excellent and good rate of the heat-sensitive blood-regulating group was higher than that of the Fenbid group(88.33%vs 68.33%,P<0.05).The VAS score,inflammatory factors(IL-6,TNF-α,hs-CRP)and blood flow index(RI)of heat-sensitive blood-regulating group and Fenbid group were lower than those before treatment(P<0.05),and the VAS score,inflammatory factors(IL-6,TNF-α,hs-CRP)and blood flow index(RI)of heat-sensitive blood-regulating group were lower than those of Fenbid group at the same time point of follow-up(P<0.05).The short-term(3 months follow-up)and longterm(6 months follow-up)blood flow indices(EDV,PSV,PI)of heat-sensitive blood-regulating group and Fenbid group were higher than those before treatment(P<0.05),and the VAS score and inflammatory factor blood flow indices(EDV,PSV,PI)of heat-sensitive blood-regulating group were higher than those of Fenbid group at the same time point of follow-up(P<0.05).Conclusion Heat-sensitive moxibustion combined with Qingre Tiaoxue Decoction can significantly improve the long-term clinical effi
