清热活血法配合速愈生肌散对肛肠术后肛周脓肿患者相关细胞因子影响  

Effect of Qingre Huoxue Method and Quyu Shengji Powder on Cytokines in Patients with Perianal Abscess After Anorectal Surgery

摘　　要：目的探究清热活血法配合速愈生肌散对肛肠术后肛周脓肿患者相关细胞因子的影响。方法选取2016年7月—2018年7月收治的肛肠术后肛周脓肿患者100例,按随机数字表法将其分为对照组(n=50)和观察组(n=50)。对照组给予清热活血法治疗,观察组在此基础上配合速愈生肌散治疗。统计两组治疗后临床疗效;比较两组患者治疗后不同时间创面愈合缩小率;比较两组患者治疗前后免疫功能指标变化情况;比较两组患者治疗后不同时间点创面肉芽组织中相关细胞因子的表达情况。结果观察组临床总有效率显著优于对照组(P<0.05);治疗后14 d,观察组创面愈合缩小率显著优于对照组(P<0.05);治疗后,观察组CD3^+、CD4^+、CD4^+/CD8^+(CD8^+)显著高(低)于对照组(P<0.05);治疗后7 d和14 d,观察组创面肉芽组织中CD68、Lyn、VEGF和SMA水平显著高于对照组(P<0.05)。结论清热活血法配合速愈生肌散可显著提高肛肠术后肛周脓肿患者免疫功能,促进创面愈合,促进创面肉芽组织中CD68、Lyn、VEGF和SMA相关细胞因子的表达,且疗效优于单独采用清热活血法治疗,值得临床推广应用。Objective To explore the effect of Qingre huoxue method and Quyu Shengji Powder on cytokines in patients with perianal abscess after anorectal surgery.Methods 100 patients with perianal abscess after anorectal surgery admitted to our hospital from July 2016 to July 2018 were randomly divided into control group(n=50)and observation group(n=50).The control group was treated with the method of clearing heat and activating blood circulation,while the observation group was treated with Quyu Shengji Powder on this basis.The clinical efficacy of the two groups after treatment was counted;the wound healing shrinkage rate at different time after treatment was compared between the two groups;the changes of immune function indexes before and after treatment were compared between the two groups;and the expression of related cytokines in wound granulation tissue at different time points after treatment was compared between the two groups.Results The total clinical effective rate of the observation group was markedly better than that of the control group(P<0.05);14 days after treatment,the wound healing shrinkage rate of the observation group was memorably better than that of the control group(P<0.05);after treatment,CD3^+,CD4^+,CD4^+/CD8^+(CD8^+)of the observation group was dramatically higher(lower)than that of the control group(P<0.05);7 days and 14 days after treatment,CD68,Lyn,VEGF and SMA of the observation group were substantially higher than those of the control group(P<0.05).Conclusion Qingre Huoxue method combined with Quyu Shengji Powder can significantly improve the immune function of patients with perianal abscess after anorectal surgery,promote wound healing,and promote the expression of CD68,Lyn,VEGF and SMA-related cytokines in granulation tissue of the wound.The curative effect is better than that of Qingre Huoxue method alone,and it is worthy of clinical application.
