消银颗粒联合阿维A胶囊治疗寻常型银屑病临床疗效和安全性的Meta分析  

Meta-analysis of Efficacy and Safety of Xiaoying Granules Combined with Acitretin Capsule in Patients with Psoriasis Vulgaris

摘　　要：目的系统评价消银颗粒联合阿维A胶囊治疗寻常型银屑病临床疗效和安全性。方法通过检索Embase、PubMed、Cochrane图书馆、中国知网、中国生物医学文献数据库、维普、万方数据库等,收集消银颗粒联合阿维A胶囊治疗寻常型银屑病的随机对照研究(RCT),采用系统评价Cochrane 5.1.0偏倚风险评估工具对最终纳入研究的方法学质量进行评价,提取数据并用Rev Man 5.3软件进行Meta分析。结果共纳入20项RCT,共涉及2303例患者,其中19项研究报告了临床总有效率、18项研究报告了临床治愈率,5项研究报告了临床复发率,4项研究报告了PASI评分指标,5项研究报告了血清炎症因子指标,15项研究报告了不良反应的发生情况,临床有效率的Meta分析结果提示:消银颗粒联合阿维A胶囊的临床总有效率高于对照组(OR=3.55,95%CI[2.76,4.57],P<0.001),消银颗粒联合阿维A胶囊的临床治愈率高于对照组(OR=2.47,95%CI[2.05,2.98],P<0.001);临床复发率的Meta分析结果提示消银颗粒联合阿维A胶囊的临床复发率低于对照组(OR=0.41,95%CI[0.24,0.69],P=0.0008);在改善PASI评分、血清炎症因子指标方面,消银颗粒联合阿维A胶囊组优于对照组,且差异显著(P<0.05),漏斗图分析提示不存在发表偏倚,敏感性分析表明结果稳定、可信。两组不良反应发生情况比较差异无统计学意义(P>0.05)。结论消银颗粒联合阿维A胶囊治疗寻常型银屑病具有较好的临床疗效,然而,确切结论有待于更多的高质量、多中心、大规模的随机对照研究证实。Objective To evaluate the benefits and side effects of Xiaoying Granules combined with Acitretin Capsule in the treatment of psoriasis vulgaris.Methods We retrieved Chinese and English databases(PubMed,Embase,the Cochrane library,CBM,VIP,Wanfang database,CNKI).The randomized controlled trials(RCTs)about therapeutic efficacy of Xiaoying Granules combined with Acitretin Capsule in the treatment of psoriasis vulgaris were collected.The methodological quality of the RCTs was evaluated based on the Cochrane Risk of Bias Assessment Tool.The data was extracted and the Meta-analysis was carried out by Reviewer Manager 5.3.Results A total of 20 RCTs with 2303 participants were included.Among these RCTs,19 articles reported the clinical total effective rate,18 articles reported the clinical cure rate,5 articles reported the clinical recurrence rate,4 articles reported the PASI scores,5 articles reported the serum inflammatory factors and 15 articles reported the adverse reactions.Meta-analysis indicated that applying Xiaoying Granules combined with Acitretin Capsule group was superior to the control group in elevating the clinical total effective rate(OR=3.55,95%CI[2.76,4.57],P<0.001).Xiaoying Granules combined with Acitretin Capsule group was superior to the control group in elevating the clinical cure rate(OR=2.47,95%CI[2.05,2.98],P<0.001).Xiaoying Granules combined with Acitretin Capsule group was superior to the control group in lowering the clinical recurrence rate(OR=0.41,95%CI[0.24,0.69],P=0.0008).Xiaoying Granules combined with Acitretin Capsule group was better ameliorating PASI score and serum inflammatory factors than the control group(P<0.05).Funnel chart analysis showed that there was no publication bias.Sensitivity analysis showed that the results of this study were stable and reliable.There was no significant difference between the two groups(P>0.05).Conclusion Xiaoying Granules combined with Acitretin Capsule shows definite clinical efficacy in the treatment of proctoptosis.However,the exact conclusion woul
