木丹颗粒改善糖尿病周围神经病变临床观察

摘　　要：背景糖尿病性神经病变是糖尿病常见的长期并发症之一,可能涉及周围神经系统和中枢神经系统,其中,糖尿病周围神经病变(DPN)是最常见的,糖尿病周围神经病变不仅可引起肢体疼痛,麻木和运动受限,还可导致由于感觉减退和感觉丧失引起的足部溃疡或截肢,严重影响糖尿病患者的生活质量并增加其死亡率。目的观察木丹颗粒对改善糖尿病周围神经病变的临床疗效。方法将100例2型糖尿病合并糖尿病周围神经病变患者于2018年1月-2018年12月按随机数字表分为对照组和治疗组,每组50例。两组均接受基础治疗,如饮食,运动和常规降糖药物,对照组的药物是常规的营养神经药物。观察组在对照组的基础上用木丹颗粒治疗12周。记录两组治疗后的VAS评分和电流感觉阈值、评估临床总有效率。结果观察组治疗后VAS评分、刺痛感,感觉减退,灼烧感明显改善,电流感觉阈值明显改善(P<0.05)。观察组总有效率也明显高于对照组(P<0.05)。结论木丹颗粒可以改善糖尿病患者周围神经病变的临床相关症状,尤其是主观感受的刺痛和灼热,提高当前电流感觉的阈值,进一步改善生活质量,这是一种安全可靠,值得临床应用的中医方法。Background Diabetic neuropathy is one of the common long-term complications of diabetes,which may involve the peripheral nervous system and central nervous system.Among them,diabetic peripheral neuropathy(DPN)is the most common diabetic neuropathy.DPN not only causes limb pain,numbness and movement restriction,but also leads to foot ulcers or amputation due to hypaesthesia and sensory loss,which seriously affects the quality of life and increases the mortality rate of diabetic patients.Objective To observe the effect of Mudan Granules on DPN.Methods A hundred patients with type 2 diabetes mellitus combined with DPN were divided into control group(n=50)and treatment group(n=50)according to random numbers from January2018 to December 2018.Both groups received basic treatment,such as diet control,exercise and oral routine hypoglycemic drugs.The drug in the control group was a routine nutritional neurodrug.The observation group was treated with Mudan Granules for 12 weeks on the basis of the control group.The VAS score and current sensing threshold of the two groups after treatment were recorded and the total clinical response rate was evaluated.Results After treatment,VAS score,tingling sensation,hypaesthesia and burning sensation were obviously improved,and the current sensation threshold was obviously improved(P<0.05).The total efficiency of the observation group was also significantly higher than that of the control group(P<0.05).Conclusion Mudan Granules can improve the clinical symptoms of diabetic patients with peripheral neuropathy,especially the subjective feelings such as tingling and burning.It can also raise the threshold of current sensation and further improve the quality of life.This is a safe and reliable method of traditional Chinese medicine which is worthy of clinical application.
