从脾论治改善功能性腹泻患者生活质量分析

摘　　要：目的探讨从脾论治对功能性腹泻患者生活质量及相关影响因素的改善。方法60例功能性腹泻患者分为治疗组和对照组,治疗前后分别采用中医症状积分表、医院焦虑与抑郁量表(HAD)、SF-36生活质量量表进行评分,进行相关统计分析。结果治疗后两组中医症状积分比较有显著性差异(P<0.05)。HADA两组组内、治疗后组间比较有显著性差异(P<0.05)。两组SF-36心理维度积分、生活质量总评分有显著性差异(P<0.05)。单因素分析得出生理维度积分在性别、病程、证候分型方面有显著性差异(P<0.05),心里维度积分在病程有显著性差异(P<0.05),生活质量总评分在性别、病程、证候分型方面有显著性差异(P<0.05)。多元直线回归方程得出病程、焦虑积分、证候积分是心理维度及生活质量总评分的危险因素;而性别、病程、焦虑积分、证候积分是生理维度的危险因素。结论从脾论治功能性腹泻能够减少中医症状积分,减少焦虑积分,对功能性腹泻患者生活质量影响因素具有改善作用。Objective To discuss the function of treating functional diarrhea from the spleen to improve the quality of life and related factors.Methods Sixty patients with functional diarrhea were divided into treatment and control groups.The score of TCM symptom table,Hospital Anxiety and Depression Scale(HAD)and SF-36 quality of life questionnaire of patients in the two groups were compared before and after treatment.Results Compared with the control group,the score of TCM symptom table of the two groups were significant(P<0.05).HAD in two groups after treatment and that in the same group before and after treatment were significant(P<0.05).The mental score and the total quality of life of SF-36 in two groups were changed significantly(P<0.05).The gender,disease duration and symptoms in two groups were significant by univariate analysis(P<0.05).Multiple linear regression equations derived the course,integral anxiety,symptom scores and quality of life were the psychological dimension total score of risk factors and gender,disease duration,anxiety integration and symptom scores were risk factors for physiological dimension.Conclusion Treating functional diarrhea from the spleen can reduce the score of TCM symptoms and relieve anxiety to improve quality of life.
