《针灸大成》腧穴配伍规律浅析

摘　　要：目的归纳总结《针灸大成》针刺处方配伍及所治疾病规律。方法以《针灸大成》作为文献检索来源,根据纳入标准与排除标准选择相关条文,建立数据库。结果共纳入针刺处方1756条,经分析得出《针灸大成》针刺处方取穴多以多穴为主,且处方配穴方法以远近配穴法为主;单穴与多穴处方均以治疗内科疾病为主;单穴所治内科疾病中以肢体经络病症为主,所治外科疾病中以疮疡及男性前阴病为主;所治妇科病中以月经病为主;所治眼耳鼻喉疾病中以口腔类疾病及眼疾为主。配穴所治内科疾病中以肢体经络病证为主;所治外科疾病中以疮疡类疾病为主;所治妇科疾病中以月经病为主;所治眼耳鼻喉类疾病中以眼科疾病为主。不同配穴方法的治疗特点为局部配穴法与局远配穴法所治疾病中以内科疾病为主。局部配穴法所治内科疾病中以治疗肢体经络病证为主,远近配穴法中以治疗气血精液病证为主。结论《针灸大成》针刺处方以多穴为主,且配穴方法以远近配穴法为主,单穴与多穴处方所治疗疾病各有侧重;不同的配穴方法所治疾病亦各有侧重。Objective To summarize the compatibility of acupuncture and moxibustion prescriptions and the rules of the diseases treated in Dacheng of Acupuncture and Moxibustion.Methods Taking Dacheng of Acupuncture and Moxibustion as a literature search source,relevant articles were selected according to the inclusion and exclusion criteria,and a database was established.Results A total of 1756 acupuncture prescriptions were includedand it was concluded that the prescriptions in Dacheng of Acupuncture and Moxibustionwere mainly composed of multiple acupoints,and the compatibility method was mainly the compatibility of distant-local acupoints.The prescriptionsof single acupoints and multiple acupoints were mainly used to treat internal diseases.The main medical diseases treated by single acupoints were limbs and meridians diseases and surgical diseases were ulcers and prepubertal diseases.The main disease in the department of gynaecology treated by single acupoints was menstruation diseases.The main diseases of the eyes,ears,nose and throat treated by single acupoints were oral diseases and eye diseases.The limbs and meridians were the main internal diseases treated by multiple acupoints.The major surgical diseases were sores.The disease of department of gynaecology treated by multiple acupoints was mainly menstruation disease.Ophthalmologic disease is the main disease in the treatment of eye,ear,nose and throat diseases.The treatment characteristics of different acupoints matching methods are mainly compatibility of distant-local acupoints.In the treatment of internal diseases,Qi,blood and semen syndromes are the main diseases treated by local and far acupoints.Conclusion The prescriptions of Dacheng of Acupuncture and Moxibustion were mainly based on multiple acupoints,and the method of acupointscompatibility was mainly local and far acupoints.Different acupointscompatibility methods have different emphasis on different diseases.
