摘 要:目的:探讨能谱CT定量参数对于浸润性肺腺癌患者评估EGFR突变情况的可行性。方法:回顾性分析经病理证实的98例肺浸润性腺癌患者临床资料,所有患者于术前行能谱CT增强扫描,并且均行EGFR基因检测。能谱定量参数包括标准化碘含量,标准化水含量,能谱曲线斜率。将患者分为EGFR突变组及EGFR野生组。结果:EGFR突变组与野生组动脉期标准化碘含量分别为0.19±0.06.0.16±0.06,动脉期能谱曲线斜率分别为2.31±0.87.1.86±0.75,两组间的差异均有统计学意义(P<0.05),EGFR突变组与野生组动脉期标准化水含量分别为0.66±0.26.0.83±0.24,静脉期的标准化水含量分别为0.66±0.27.0.84±0.25,两组间的差异有统计学意义(P<0.05),静脉期的标准化水含量评估EGFR突变情况曲线下面积为0.713。结论:在浸润性肺腺癌中,能谱CT定量参数对于预测EGFR基因突变状态具有一定价值。Purpose:To explore the feasibility of spectral CT quantitative parameters in evaluating epidermal growth factor receptor mutation in patients with invasive pulmonary adenocarcinoma.Methods:Ninety-eight patients with invasive pulmonary adenocarcinoma confirmed by pathology results were enrolled in this study.All patients underwent enhancement scan with spectral CT before surgical tumor resection and with EGFR gene detection after operation.The quantitative parameters of spectral CT included standardized iodine content,standardized water content,slope of energy spectrum curve.Patients were divided into EGFR mutation group and EGFR wild type group.Results:The normalized iodine content in arterial phase in the EGFR mutation group and the wild type group were 0.19±0.06 and 0.16±0.06,respectively.The slopes of the arterial phase spectrum curves were 2.31±0.87 and 1.86±0.75,respectively.The differences between the groups were with statistical significant(P<0.05).The normalized water content in arterial phase in the EGFR mutation group and the wild group were 0.66±0.26 and 0.83±0.24,respectively.The standardized water content in the venous phase was 0.66±0.27 and 0.84±0.25,respectively.The difference between the twq groups was with statistical significant(P<0.05).Using the standardized water content in the venous to evaluate the EGFR mutations,the area under the curve was 0.713.Conclusion:The quantitative parameters of spectral CT have certain value for predicting the mutation status of EGFR in invasive pulmonary adenocarcinoma.
