摘 要:目的:评价垂直血流(TP)和平行血流(IP)相位对比(PC)MRI测量先天性心脏病(CHD)血流流速峰值的准确性。方法:本研究包括25例伴有瓣膜或血管狭窄术前或术后的患者,年龄平均7.35+4.34岁(10个月~15岁)。应用TP-PC和IP-PCMRI测量瓣膜或血管狭窄部位血流的最大流速,并与同期的多普勒(Doppler)检出结果比较,应用One-Way Anova和配对t检验进行统计学分析。结果:Doppler最大流速测量值为(329.48+97.23cm/s),TP-PC和IP-PC测量值分别为(297.48士118.04cm/s)和(319.28士113.20 cm/s),三者比较未见显著性差异(F=0.55,P>0.05);但两两比较显示仅IP-PC MRI与Doppler测量值未见显著性差异(P>0.05)。结论:IP-PC和TP-PC MRI均可以同Doppler一样准确测定CHD中狭窄部位的血流流速峰值,但IP-PCMRI测量结果更接近于Doppler.Purpose:To evaluate the accuracy of phase-contrast MRI by using in-plane and through-plane manner on measuring the flow peak velocities in patients with congenital heart disease(CHD),and to compare the result with Doppler sonography.Methods:Twenty-five patients with semilunar valve or great artery stenosis pre-or post-operation(mean age 7.35±4.34 years)were included in this study.Flow peak velocities of narrowing vessel or semilunar valve were measured by in-plane PC-MR aligned along the flow jet and through plane PC-MR perpendicular to the flow direction.One-Way ANOVA and paired T-test were used for statistical comparison between in-plane and through-plane phase-contrast MRI,and Doppler sonography.Results:Peak velocity measurement was the largest for Doppler sonography(329.48±97.23cm/s cm/s),followed by in-plane phase-contrast MRI(319.28±113.20cm/s),and the smallest for through-plane phase contrast MRI(297.48±118.04cm/s).However,flow peak velocities measured by in-plane and through plane phase contrast MRI,Doppler sonographer were shown with no statistical significant difference(P>0.05).There was no statistical significant diference of peak velocity measurements between in-plane phase contrast MRI and Doppler sonography(P>0.05).But significant difference was observed between through-plane phase-contrast MRI and Doppler sonography(P<0.05).Conclusion:Both in-plane and through-plane phase-contrast MRI can provide as accurate measurerments of flow peak velocity as Doppler Sonography in patients with CHD.But in-plane phase-contrast MRI measurements were closer to Doppler Sonography.
