祛寒定痛饮加味联合西药对强直性脊柱炎肾虚督寒证患者疗效及BASFI、BASMI评分的影响

摘　　要：目的:观察祛寒定痛饮加味联合西药对强直性脊柱炎(AS)肾虚督寒证患者疗效及巴氏AS功能指数(BASFI)、巴氏AS计量指数(BASMI)评分的影响。方法:随机数表法将本院2016年1月~2019年1月诊治的96例AS肾虚督寒证患者分为观察组与对照组,各48例,对照组给予双氯芬酸钠、柳氮磺吡啶治疗,观察组治疗方法是在此基础上加用祛寒定痛饮。观察两组总有效率、不良反应,还将两组临床症状(胸廓活动度、晨僵时间、Schober试验、疼痛关节数)、中医证候积分、炎症相关指标[C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)、血沉(ESR)]和BASFI、BASMI评分的治疗前后进行对比。结果:观察组治疗总有效率93.75%,明显高于对照组79.17%(P<0.05);观察组治疗后胸廓活动度、晨僵时间、Schober试验、疼痛关节数、CRP、TNF-α、ESR、中医证候积分、BASFI和BASMI评分均优于对照组(P<0.05);两组不良反应发生率对比无明显差异(P>0.05)。结论:驱寒定痛饮加味联合西药治疗强直性脊柱炎肾虚督寒证的疗效好,安全性高,能明显改善BASFI、BASMI评分。Objective:To study the curative effect of Modified Quhan Dingtong Decoction combined with western medicine for patients with ankylosing spondylitis(AS) of kidney-deficiency Du-cold syndrome and its influence on scores of Bath AS functional index(BASFI) and Bath AS measures index(BASMI).Methods:A total of 96 AS patients of kidney-deficiency Du-cold syndrome who were diagnosed and treated in the hospital from January 2016 to January 2019 were randomly divided into observation group and controlled group, 48 cases in each group.The controlled group was treated with Diclofenac Sodium and Sulfasalazine, while the observation group was additionally treated with Modified Quhan Dingtong Decoction.The total response rates and adverse reactions in both groups were observed.The clinical symptoms(thorax activity, morning stiffness time, Schober test, number of painful joints), scores of TCM syndromes, inflammation-related indexes [C reactive protein(CRP), tumors Necrosis factor-α(TNF-α), erythrocyte sedimentation(ESR)], scores of BASFI and BASMI before and after treatment were compared between the two groups.Results:The total response rate of treatment in the observation group was significantly higher than that in the controlled group(93.75% vs 79.17%)(P<0.05).After treatment, thoracic activity, morning stiffness time, Schober test, number of painful joints, CRP, TNF-α, ESR, scores of TCM syndromes, BASFI and BASMI in the observation group were better than those in the controlled group(P<0.05).There was no significant difference in incidence of adverse reactions between the two groups(P>0.05).Conclusion:The curative effect of Modified Quhan Dingtong Decoction combined with western medicine is good on AS of kidney-deficiency Du-cold syndrome, with high safety.It can significantly improve scores of BASFI and BASMI.
