益肾生血汤改善慢性肾性贫血的临床疗效观察及对重组人促红细胞生成素的调节作用

摘　　要：目的:观察益肾生血汤治疗慢性肾性贫血的临床疗效观察及对重组人促红细胞生成素的调节作用。方法:随机选取慢性肾性贫血患者82例,随机分为对照组及观察组,每组各41例。2组患者均采用基础治疗,观察组在此基础上加以益肾生血汤治疗,2组均治疗3个疗程。比较两组患者治疗前后的中医症候及体征积分、肾功能指标及贫血指标。比较2组患者的临床疗效。结果:观察组临床总有效率95.1%,对照组总有效率82.9%,观察组明显优于对照组(P<0.05)。治疗后两组Scr、BUN水平均较本组治疗前比较显著降低(P<0.05),GFR较本组治疗前比较显著升高(P<0.05),观察组治疗后的Scr、BUN、GFR水平变化幅度明显高于对照组(P<0.05)。治疗后两组的HCT、RBC、Hb、SF及EPO的水平均显著升高(P<0.05),与对照组比较,观察组治疗后的HCT、RBC、Hb、SF及EPO水平显著高于对照组,比较有统计学意义(P<0.05)。两组患者治疗后倦怠乏力、少气懒言、脘腹胀满、腰膝酸软及食少纳呆等各项中医症候及体征积分均显著降低(P<0.05),并且治疗后观察组的各项积分均显著较对照组低(P<0.05)。结论:益肾生血汤可以显著提高患者的临床治疗效果,对重组人促红细胞生成素具有较好的调节作用。Objective:To observe the clinical effect of Yishen Shengxue Decoction on chronic renal anemia and its regulation on recombinant human erythropoietin.Methods:82 patients with chronic renal anemia were randomly selected and randomly divided into controlled group and observation group, 41 cases in each group.The two groups were treated with basic therapy, while the observation group was treated with Yishen Shengxue Decoction on the basis of basic treatment.Both groups were treated for three courses.The scores of TCM symptoms and signs, renal function indexes and anemia indexes before and after treatment were compared between the two groups.The clinical effects of the two groups were compared.Results:The total clinical effective rate of the observation group was 95.1%, and that of the controlled group was 82.9%.The observation group was significantly better than that of the controlled group(P<0.05).After treatment, the levels of Scr and BUN in the two groups were significantly lower than those before treatment(P<0.05), and the levels of GFR were significantly higher than those before treatment(P<0.05).The levels of SCr, BUN and GFR in the observation group were significantly higher than those in the controlled group(P<0.05).After treatment, the levels of HCT, RBC, Hb, SF and EPO in the two groups were significantly higher than those in the controlled group(P<0.05).Compared with the controlled group, the levels of HCT, RBC, Hb, SF and EPO in the observation group were significantly higher than those in the controlled group(P<0.05).After treatment, the scores of TCM symptoms and signs such as fatigue, lack of breath and laziness, abdominal distention, weakness of waist and knee, and lack of appetite in both groups were significantly reduced(P<0.05), and the scores in the observation group were significantly lower than those in the controlled group(P<0.05).Conclusion:Yishen Shengxue Decoction can significantly improve the clinical efficacy of patients, and has a good regulatory effect on recombinant human erythropoietin
