摘 要:目的对基于EQ-5D健康测量获取失能权重方法进行系统评价。方法计算机检索PubMed、EMbase、Web of Science、CNKI和WanFang Data数据库,搜集基于EQ-5D健康测量获取失能权重方法的研究,检索时限均从建库至2019年6月1日。由2名研究者独立筛选文献、提取资料和评价纳入研究的偏倚风险后,对EQ-5D健康测量获取失能权重方法进行系统评价。结果最终纳入18个研究,均发表于2003~2018年。纳入研究涉及疾病多样,内容多集中在疾病相关生活质量和疾病负担。纳入研究显示基于EQ-5D健康测量获取失能权重的方法包括:"失能权重=健康效用值普通人群或对照组-健康效用值疾病"(7个)、"失能权重=1-VAS得分/100"(6个)、"失能权重=1-健康效用值疾病"(3个)、线性回归模型(1个)和映射法(1个)。结论 EQ-5D相关失能权重测算方法涉及疾病多样,可比性一般,多数相关方法探索来自国外研究。以普通人群或对照组与疾病组的健康效用值相减的失能权重测算方法最常见。Objectives To systematically review approaches to derive disability weights(DWs) based on EQ-5D instrument. Methods PubMed, EMbase, Web of Science, CNKI and WanFang Data databases were electronically searched to collect studies on the approaches to derive disability weights based on EQ-5D from inception to June 1st, 2019.Two reviewers independently screened literature, extracted the basic information and evaluated risk of bias of included studies. Then, systematic review on approaches to derive DWs based on EQ-5D instrument was performed. Results A total of 18 studies were included, which were published between 2003 and 2018. The included studies involved a variety of diseases, mostly focusing on quality of life and the burden of disease. The approaches to derive DWs based on EQ-5D health instrument were as follows: DWs=health utility scoreNormal or Control-health utility scoreDisease(7 studies), DWs=1-VAS score/100(6 studies), DWs=1-health utility scoreDisease(3 studies), linear regression model(1 study), and mapping(1 study). Conclusions Among all the included studies using EQ-5D-based disability weight measurement methods involves a variety of diseases, with relatively low comparability. More methodological studies are from abroad. Among all the applied approaches, DWs equally to health utility scoreNormal or Control minus health utility scoreDisease is the most commonly used.
