介入永久栓塞治疗大咯血的研究及责任血管的分析

摘　　要：目的 探讨永久性栓塞颗粒介入栓塞支气管动脉治疗大咯血的疗效及咯血责任血管的分布.方法 回顾性分析行支气管动脉栓塞(BAE)治疗大咯血的患者68例影像、临床资料.结果 68例患者均成功进行BAE治疗大咯血,所有患者均达到即刻止血,即刻止血率100%,总体有效率95.59%.血管造影异常表现:血管迂曲83支,增粗56支,紊乱45处,异常染色23处,对比剂外溢14处,动静脉畸形2例,动静脉瘘1例.共发现责任血管151支,其中支气管动脉118支,胸廓内动脉15支,肋间动脉14支,锁骨下动脉无名分支2支,甲状颈干1支,膈下动脉1支.结论 BAE治疗大咯血效果显著,安全有效,支气管动脉、胸廓内动脉、肋间动脉是主要责任血管.Objective To evaluate the curative effect of permanent embolization granule in embolizing the bronchial artery in the treatment of massive hemopoysis and to investigative the disribution of blood vessels responsible for hemoptysis.Methods The imaging and clinical data of 68 patients with massive hemoptysis treated by bronchial artery embolization(BAE)were analyzed retrospectively.Results BAE was used to treat all 68 massive hemoptysis patientse successfully,and all of them achieved immediate hemostasis with 100% hemostasis rate and 95.59% overall effective rate,Abnormal angiographic manifestations included 83 vessels tortuosty,56 vessels enlargerment,45 disorders,23 abnormal staining,14 contrast extravasation,2 arteriovenous malformations and 1 arteriovenous fistula.151 responsible blood vessels were found totally,including 118 bronchial arteries,15 internal thoracic arteries,14 intercostal arteries,2 innominate branches of subclavian arteres,1 thyrocervical trunk and 1 infeior phrenic artery.Conclusion BAE is effective and safe in the treatment of massive hemoptysis.Bronchial artery,internal thoracic artery and intercostal artery are the main responsible vessels.
