摘 要:目的 探讨MR IVIM技术应用于贵州小型猪心脏检查的可行性.方法 选择健康贵州小型猪8只,采用快速稳态梯度回波(FIESTA)序列进行标准二、三、四腔心以及左室短轴(覆盖心尖部到心底部)电影成像.在左心室短轴层面,分别对左心室心尖部、心中部以及心底部进行IVIM成像.由2名从事心血管影像诊断的高年资医师采取双盲法对心脏IVIM图像质量等级进行评估,其中较高年资者间隔1个月以上对相关定量参数进行再次测量,对参数的可重复性和一致性进行组内相关系数(ICC)分析.结果 总计24层心肌短轴IVIM图像,其中6层图像质量不能达到诊断要求,18层图像质量达到Ⅱ级或Ⅲ级图像质量要求,总成功率75.0%(18/24).图像分析者间的Kappa系数值在IVIM图像上为0.79,图像质量评估一致性较好.同一名医师重复2次定量测量以及2名医师分别定量测量左室心肌所得各参数的一致性较好(ICC>0.75).结论 MR IVIM成像图像质量等级评估一致性较好,成功率较高.各参数定量测量的重复性及一致性较好,IVIM成像技术用于小型猪心脏扫描具有可行性.Objective To investigate the feasibility of the application of the technique of the IVIM in the heart examination of Guizhou miniature pigs.Methods Standard two-chamber,three-chamber,four-chamber and left ventricular short-axis(covering the apex of the heart to the bottom of the heart)imaging was performed on 8 miniature pigs using FIESTA.IVIM imaging was performed on the left ventricular apex,the heart,and the bottom of the heart at the level of the left ventrice brachyaxis.The quality level of cardiac IVIM images was assessed by a double-blind method by 2 seniot physicians engaged in cardiovascular imaging diagnosis.The relevant quantitative parameters were measured again by the senior physicians at intervals of more than 1 month,and the repeatability and consistency of the parameters were analyzed by intraclass correlation coefficient(ICC).Results A total of 24 layers of short axis IVIM images of the ventricle,of which 6 layers could not meet the diagnostic requirements,18 layers meet the level Ⅱ or Ⅲ image quality requirements,and the total success rate was 75.0%(18/24).The Kappa coefficient among image analysts was 0.79 in IVIM images,indicating a good consistency in image quality assessment.The consistency of the parameters obtained by repeated quantitative measurements twice by the same physician and quantitative measurements of left ventricular myocardium by two physicians was good(ICC>0.75).Conclusion The evaluation of MR IVIM imaging quality grade has good consistency and high success rate.The quantitative measurement of each parameter has good repatability and consistency,and IVIM imaging technology is feasible for the heart scanning of miniature pigs.
