摘 要:目的应用斑点追踪技术评价慢性肾病(CKD)患者的心房机械功能,探讨左房机械功能与肾功能相关性。方法分别入组对照组30例、CKD2~4级33例,CKD5级患者35例,采集心房四腔心和两腔心心房二维图像,脱机分析心房峰值应变(LAPS),导管功能应变(LACS)和泵功能应变(LABPS),分析左房机械功能与肾功能相关性。结果三种应变在终末期肾病中比对照组均明显减低(P<0.05),且LAPS和LABPS与eGFR独立相关(P<0.05)。结论慢性肾病患者的左房机械功能与肾功能相关,其中左房峰值应变和泵功能应变与肾功能独立相关。Objective To evaluate the atrial mechanics in patients with chronic kidney disease(CKD)and to explore the correlation between left atrial mechanical function and renal function.Methods Thirty-three patients with CKD stage 2-4,35 patients with CKD stage 5 and 30 control subjects were enrolled.Two dimensional images left atrium were collected on four-chamber and two-chamber view.Left atrial peak longitudinal strain(LAPS),left atrial conduit strain(LACS),and left atrial booster pump strain(LABPS)were analyzed using offline soft wear.The associations between left mechanic parameters and renal function were analyzed.Results Left atrial strain were significantly decreased in patients with CKD stage 5(P<0.05).LAPS and LABPS were correlated with eGFR(P<0.05).Conclusions Left atrial strain parameters are independently associated with renal function in patients with chronic kidney disease.
