腱鞘巨细胞瘤与腱鞘纤维瘤超声表现分析

摘　　要：目的分析腱鞘巨细胞瘤与腱鞘纤维瘤超声表现特征。方法回顾性分析了经手术和病理学诊断的25例腱鞘巨细胞瘤和12例腱鞘纤维瘤的患者,术前均进行了超声检查,记录肿物的位置、内部回声、大小、形态、边界、有无钙化、有无液化、和周围组织的关系、血流分级等情况。应用统计学分析其特征差异。结果25例病理确诊的腱鞘巨细胞瘤常表现为肌腱旁、关节周围或包绕肌腱的实性肿物,回声多不均匀,形态规则或不规则,边界清晰,可伴有钙化和液化,12例病理证实为腱鞘纤维瘤位于肌腱旁和关节腔内,实性或囊实性,形态多规则,边界清晰,未发现钙化。腱鞘巨细胞瘤与腱鞘纤维瘤超声表现位置、形态、边界钙化及液化等超声表现差异无统计学意义(P>0.05)。结论GCTTS和FTS超声表现相似,但有一定的倾向性,包绕肌腱生长的不规则、不均匀伴钙化的肿物更倾向于诊断为GCTTS,肌腱旁或关节旁的规整的、均匀的实性肿物更倾向于FTS。Objective To analyze the ultrasonographic features of giant cell tumor and fibroma of tendon sheath.Methods 25 cases of giant cell tumor of tendon sheath and 12 cases of fibrous tumor of tendon sheath diagnosed by operation and pathology were analyzed retrospectively.Before operation,ultrasound examination was carried out to record the position,internal echo,size,shape,boundary,calcification,liquefaction,relationship with surrounding tissue,blood flow classification and so on.The difference of the characteristics was analyzed by statistics.Results 25 cases of giant cell tumor of tendon sheath confirmed by pathology often showed solid mass beside tendon,around joint or surrounding tendon,with irregular echo,regular or irregular shape,clear boundary,calcification and liquefaction.12 cases of giant cell tumor of tendon sheath confirmed by pathology were located in tendon side and joint cavity,solid or cystic solid,regular shape,clear boundary,without calcification.There was no significant difference between giant cell tumor and fibroma(P>0.05).Conclusions The ultrasound of GCTTS and FTs are similar,but there,s a certain tendency,irregular uneven swellings that wrap around tendon growth with calcification tend to be diagnosed as GCTTS,uniform solid swollen on the side of the tendon or joint tend to FTS.
