摘　　要：目的探讨胰腺炎胰周血管并发症的超声声像特点及诊断价值。方法回顾性分析16例急、慢性胰腺炎胰周血管并发症患者的超声特征。结果假性动脉瘤7例(7/16),彩色多普勒超声显示瘤体内有红蓝相间漩涡状彩色血流及双期双向血流频谱。当瘤体发生破裂时血肿表现为无血流信号的不规则、非均质回声,并伴有腹腔积液。静脉血栓形成占9/16,其中4/9合并胰源性门静脉高压。结论胰腺炎可引起多种胰周血管并发症,超声对诊断胰腺炎血管并发症具有重要的临床价值。Objective To investigate the ultrasonic characteristic and diagnostic value of vascular complications in pancreatitis.Methods The ultrasonogram of 16 cases which were confirmed by operation,DSA and enhanced CT were analyzed retrospectively.Results In our study,7/16 patients occurred pseudoaneurysm(the splenic artery,superior mesenteric artery and celiac trunk artery involved).Pseudoaneurysm can well be diagnosed by color Doppler ultrasonography with charateristics of to-and-fro spectrum and swirl flow.Haematoma was found as an irregular heterogeneous echo zone with ascites in ruptured pseudoaneurysms(2/7 patients),no blood flow signals were observed.The venous thrombosis occurred in 9/16.4/9 patients were identified as pancreatic portal hypertension and splenomegaly.Conclusions Ultrasonography has significant clinical value in diagnose various peripancreatic vascular disorders.
