针灸疗法对带状疱疹后遗神经痛患者血清细胞因子和疼痛因子水平的影响

摘　　要：【目的】观察针灸治疗带状疱疹后遗神经痛的临床疗效,并探讨其可能的作用机制。【方法】将120例带状疱疹后遗神经痛患者随机分为观察组和对照组,每组各60例,对照组给予常规西药治疗,观察组在对照组基础上,同时给予针灸治疗。连续治疗4周。治疗4周后,比较2组患者治疗前后的疼痛视觉模拟评分(VAS)、白细胞介素6(IL-6)、肿瘤坏死因子α(TNF-α)、白细胞介素1β(IL-1β)、白细胞介素10(IL-10)、5羟色胺(5-HT)以及P物质水平的变化情况,并评价2组的临床疗效。【结果】(1)治疗2周和4周后,2组患者的VAS评分明显改善(P<0.05),且观察组在改善VAS评分方面明显优于对照组,差异有统计学意义(P<0.05)。(2)治疗后,2组患者的血清TNF-α、IL-6、IL-1β、IL-10水平明显改善(P<0.05),且观察组在改善血清TNF-α、IL-6、IL-1β、IL-10水平方面均明显优于对照组,差异有统计学意义(P<0.05)。(3)治疗后,2组患者的5-HT、P物质水平均明显改善(P<0.05),且观察组在改善5-HT、P物质水平方面均明显优于对照组,差异有统计学意义(P<0.05)。(4)观察组总有效率为96.67%(58/60),对照组为91.67%(55/60)。观察组疗效优于对照组,差异有统计学意义(P<0.05)。【结论】针灸治疗带状疱疹后遗神经痛,能明显改善患者的疼痛症状,进一步降低疼痛相关细胞因子、疼痛因子的水平,临床疗效显著。Objective To observe the clinical efficacy of acupuncture-moxibustion for the treatment of postherpetic neuralgina patients,and to explore its possible mechanism.Methods A total of 120 patients with postherpetic neuralgina were randomly divided into observation group and control group,60 cases in each group.The two groups received conventional western medicine treatment,additionally,the observation group received acupuncture-moxibustion treatment.The treatment lasted 4 continueus weeks.After 4 weeks,the visual analogue scale(VAS)pain scores and levels of interleukin 6(IL-6),tumor necrosis factorα(TNF-α),interleukin 1β(IL-1β),interleukin 10(IL-10),5-hydroxytryptamine(5-HT)and substance P were compared in the two groups before and after treatment,and the clinical efficacy was evaluated.Results(1)After 2-,4-week treatment,the VAS scores of the two groups were improved in the two groups(P<0.05),and the improvement in VAS scores of the observation group was superior to that of the control group,the difference being significant(P<0.05).(2)After treatment,the serum levels of TNF-α,IL-6,IL-1β,IL-10 of the two groups were improved in the two groups(P<0.05),and the improvement in serum levels of TNF-α,IL-6,IL-1β,IL-10 of the observation group was superior to that of the control group,the difference being significant(P<0.05).(3)After treatment,the levels of 5-HT and substance P of the two groups were improved in the two groups(P<0.05),and the improvement in the levels of 5-HT and substance P of the observation group was superior to that of the control group,the difference being significant(P<0.05).(4)The total effective rate was 96.67%(58/60)in the observation group,and was 91.67%(55/60)in the control group.The clinical efficacy of the observation group was superior to that of the control group,the difference being significant(P<0.05).Conclusion Acupuncturemoxibustion has clinical efficacy of post-herpetic neuralgina patients,can improve the pain symptom and further reduce the levels of pain-related cytokines and pa
