骨伤康复外洗颗粒对肘关节恐怖三联征术后功能恢复的临床效果研究

摘　　要：【目的】观察本院制剂骨伤康复外洗颗粒治疗肘关节恐怖三联征(指肘关节后脱位、桡骨小头骨折以及尺骨冠状突骨折的一种复杂性肘关节损伤)术后的临床疗效。【方法】将60例肘关节恐怖三联征患者随机分为熏洗组和非熏洗组,每组各30例。2组患者均在肘关节消肿后行切开复位钢板螺钉内固定术治疗及术后常规功能锻炼,在此基础上,熏洗组患者在术后14 d开始加用骨伤康复外洗颗粒熏洗患肘,非熏洗组未给予中药熏洗治疗,疗程为3周,并于术后第3个月及第6个月进行随访,观察2组患者的肘关节活动度、Brobery-Morrey评分及并发症发生情况。【结果】(1)60例患者中,熏洗组有3例脱落,非熏洗组有5例脱落,其余52例患者能坚持试验并获得足够随访(随访率86.7%),其中熏洗组27例,非熏洗组25例。(2)在术后第3个月和第6个月随访时,熏洗组患者肘关节的屈伸活动度、旋转活动度和Brobery-Morrey评分均较非熏洗组明显提高,差异均有统计学意义(P<0.05或P<0.01)。(3)末次随访时,熏洗组患者Brobery-Morrey评分的优秀率为70.4%(19/27),明显高于非熏洗组的40.0%(10/25),差异有统计学意义(P<0.05)。(4)熏洗组患者的并发症发生率为7.4%(2/27),明显低于非熏洗组的32.0%(8/25),差异有统计学意义(P<0.05)。【结论】骨伤康复外洗颗粒熏洗可明显改善肘关节恐怖三联征术后患者的肘关节功能,并能减少并发症的发生。Objective To investigate the clinical effect of Traumatic Rehabilitation Granules for Washing on the postoperative functional recovery of patients with terrible triad of the elbow, a complicated injury including posterior dislocation of the elbow joint,fractures of captulum radii,and coronoid fracture. Methods Sixty patients with terrible triad of the elbow were divided into fumigation group and non-fumigation group,30 cases in each group. All of the patients of the two groups underwent open reduction and internal fixation with plate and screw after subsiding the swelling of the elbow, and performed postoperative conventional functional training. And additionally,the fumigation group was given fumigation with Traumatic Rehabilitation Granules for Washing 14 days after the operation,and the non-fumigation group had no fumigation. The course of treatment lasted for 3 weeks. The patients were followed-up 3 and 6 months after the operation for the observation of the range of movement of the elbow,Brobery-Morrey scores and the incidence of the complications. Results(1)Of the 60 cases,3 cases from the fumigation group and 5 cases from the non-fumigation group dropped out,and a total of52 patients,27 in the fumigation group and 25 in the control group,were included for the trial and were followedup,the follow-up rate being 86.7%.(2)The results of the follow-up 3 and 6 months after the operation showed that the range of elbow flexion-extension,the range of elbow rotation,Brobery-Morrey scores in the fumigation group were obviously increased in comparison with those in the non-fumigation group, the difference being statistically significant(P<0.05 or P<0.01).(3)The results of the last follow-up showed that the excellent rate for the improvement of Brobery-Morrey scores in the fumigation group was 70.4%(19/27),and was much higher than that in the and non-fumigation group(40.0%,10/25),the difference being statistically significant(P<0.05).(4)The incidence of the complications in the fumigation group was 7.4%(2/27),and was m
