摘  要:目的研究骨导分频率听性脑干反应(BC-chirp-ABR)和骨导多频听觉稳态反应(BC-ASSR)与骨导纯音听阈(BC-PTA)在传导性听力损失患者中的差异和相关性,为临床客观听力诊断及预估主观测听阈值提供参考。方法对40例单侧传导性听力损失患者分别行掩蔽后的BC-chirp-ABR和BC-ASSR测试,获得反应阈,并与BC-PTA阈值进行相关性分析。结果在0.5、1、2、4 kHz频率,BC-chirp-ABR反应阈分别为(25.0±8.2)、(22.3±6.1)、(18.2±7.6)、(19.7±6.9)dB nHL,BC-ASSR反应阈分别为(30.7±10.1)、(28.3±9.7)、(27.7±9.1)、(27.1±8.6)dB nHL。在上述4个频率,BC-PTA阈值分别为(9.5±6.3)、(10.7±5.7)、(12.8±9.1)、(15.3±4.5)dB HL。Pearson相关性分析显示,BC-chirp-ABR和BC-ASSR反应阈与同频率BC-PTA阈值均具有显著相关性,频率越高相关性越强。结论BC-chirp-ABR和BC-ASSR能够反映同频率BC-PTA阈值,BC-chirp-ABR和BC-ASSR是较好评估主观听阈的频率特异性客观测听技术。Objective To evaluate the difference and the correlationship between bone conduction chirp evoked auditory brainstem response(BC-chirp-ABR),bone conduction auditory steady-state response(BC-ASSR)thresholds and bone conduction pure tone auditory(BC-PTA)thresholds,and to set up assessments for objective audiometry in patients with conductive hearing loss.Methods BC-chirp-ABR and BC-ASSR were performed in 40 patients with conductive hearing loss.The evoked-potential thresholds were obtained and compared with BC-PTA thresholds.Results At the frequents of 0.5,1,2,4 kHz,the thresholds of BC-chirp-ABR were 25.0±8.2,22.3±6.1,18.2±7.6,19.7±6.9 dB nHL;whereas the thresholds of BC-ASSR were 30.7±10.1,28.3±9.7,27.7±9.1,27.1±8.6 dB nHL,respectively.At the above 4 frequents,the thresholds of BC-PTA were 9.5±6.3,10.7±5.7,2.8±9.1,15.3±4.5 dB HL.Pearson correlationship analysis suggested that BC-chirp-ABR and BC-ASSR thresholds had high correlations with BC-PTA thresholds at each frequent,and higher correlations were detected at higher frequents.Conclusions BC-chirp-ABR and BC-ASSR provided rationally accurate predictions of multi-frequencies objective audiometry by taking the accuracy predicted BC-PTA thresholds.
