老年长期留置鼻胃管患者的胃内压变化与反流之间的相关研究

摘　　要：目的动态监测老年患者留置鼻胃管的不同时间段的胃内压的变化,探讨预防长期留置鼻饲管老年患者反流误吸的适宜胃内压。方法采取前瞻性自身对照研究,对20例次老年患者留置鼻胃管的不同时间段(≤1个月、2-6个月、>6个月)进行胃内压监测及咽后壁分泌物PH测定值。结果在留置鼻胃管的老年患者中,随着留置时间的增加,胃内压降低(7.40±0.59 cmH_2O、7.25±0.64 cmH_2O&9.78±3.65 cmH_2O、10.06±3.50 cmH_2O),却增加了胃内容物反流的风险(48.69%&25.2%)。结论鼻胃管的留置时间对胃内压有影响,同时也增加了胃内容的反流风险。objective to dynamically monitor the changes of gastric internal pressure during different periods of the indwelling nasogastric tube in elderly patients,and to explore the appropriate gastric internal pressure to prevent reflux aspiration in elderly patients with long-term indwelling nasogastric tube.Methods a prospective self-controlled study was conducted to monitor the internal gastric pressure and determine the PH value of the secretions in the posterior pharyngeal wall of 20 elderly patients during different periods(≤1 month,2-6 months,and 6 months of>)of indwelling nasogastric tube.Results in the elderly patients with nasogastric tube indwelling,with the increase of indwelling time,the gastric internal pressure decreased(7.40±0.59 cmH2O,7.25±0.64 cmH2O&9.78±3.65 cmH2O,10.06±3.50 cmH2O),but the risk of gastric content reflux increased(48.69%&25.2%).Conclusion the indwelling time of nasogastric tube has an effect on gastric internal pressure,and also increases the risk of gastric reflux.
