全髋关节置换术治疗强直性脊柱炎髋关节骨性强直的临床分析

摘　　要：目的探讨全髋关节置换术治疗强直性脊柱炎髋关节骨性强直的临床效果,从而为强直性脊柱炎髋关节骨性强直的治疗提供理论支持。方法选取2018年9月—2019年9月期间在该院风湿免疫科进行治疗的27例患有强直性脊柱炎的患者作为研究对象,并对患者采取全髋关节置换术进行治疗,统计治疗前后患者的治疗总有效率及髋关节活动度的变化,从而探讨全髋关节置换术治疗强直性脊柱炎髋关节骨性强直的临床效果。结果治疗后患者的临床治疗总有效率明显高于治疗前,差异有统计学意义(P<0.05);治疗后患者的Harris评分(72.6±5.8)分明显高于治疗前(31.2±8.1)分,差异有统计学意义(t=18.760,P<0.01);治疗后患者的髋关节总活动度(160.1±13.5)°明显高于治疗前(66.4±13.7)°,差异有统计学意义(t=25.190,P<0.01)。结论对强直性脊柱炎患者全髋关节置换术可有效提高其临床治疗有效率,改善患者髋关节骨性强直,疗效显著,并且预后良好。Objective To explore the clinical effect of total hip arthroplasty for ankylosing spondylitis and hip bone ankylosing,so as to provide theoretical support for the treatment of ankylosing spondylitis and hip bone ankylosing.Methods Twenty-seven patients with ankylosing spondylitis who were treated in the rheumatology and immunology department of the hospital from September 2018 to September 2019 were selected as the research subjects.The patients were treated with total hip replacement surgery and statistical treatment was performed.The total effective rate of treatment and the change of hip joint activity before and after the patients were discussed,so as to explore the clinical effect of total hip arthroplasty for ankylosing spondylitis hip ankylosing.Results The total effective rate of clinical treatment of patients after treatment was significantly higher than that before treatment,and the difference was statistically significant(P<0.05);the Hanris score(72.6±5.8)points of patients after treatment was significantly higher than that before treatment(31.2±8.1) points,the difference was statistically significant(t=18.760,P<0.01);the total hip joint mobility(160.1±13.5)° of patients after treatment was significantly higher than that before treatment(66.4±13.7)°,the difference was statistically significant(t=25.190,P<0.01).Conclusion Total hip arthroplasty for patients with ankylosing spondylitis can effectively improve the clinical treatment efficiency,improve the patients hip ankylosing,have a significant effect,and have a good prognosis.It is worth applying to clinical treatment and promoting it.
