摘　　要：目的探讨血清降钙素原(PCT)在鉴别小儿不典型化脓性脑膜炎和病毒性脑炎中的作用。方法回顾性分析本院接诊的72例脑膜炎患儿的诊疗情况,按照不同病因将36例不典型化脓性脑膜炎患儿纳入A组36例病毒性脑炎患儿纳入B组。检测2组患儿的PCT、C反应蛋白(CRP)和白细胞计数(WBC)水平,并比较2组CRP、PCT和WBC的检出率以及不同PCT水平时患儿分布情况。结果2组患儿的CRP、WBC水平及其检出率相比,差异无统计学意义(P>0.05);A组PCT水平及检出率均显著高于B组(P<0.05)。PCT检测结果表明,A组患儿占比随PCT浓度的增加而升高,B组患儿占比随PCT浓度增加而降低。结论PCT是鉴别小儿不典型化脓性脑膜炎、病毒性脑炎的理想指标,且操作简便,灵敏度高。Objective To study the role of serum procalcitonin (PCT) in distinguishing atypical suppurative meningitis from viral encephalitis in children. Methods The diagnosis and treatment of 72 children with meningitis in our hospital were retrospectively analyzed. According to different etiologies,36 children with atypical suppurative meningitis were selected in group A and 36 children with viral encephalitis were in group B. The levels of PCT,C reactive protein (CRP) and leukocyte count (WBC) were measured and analyzed,and their detection rates,as well as the distribution of children in different PCT concentrations were compared between the two groups. Results There were no significant differences in CRP and WBC levels as well as their detection rates between the two groups (P > 0. 05). PCT level and its detection rate in group A were significantly higher than that in group B (P < 0. 05). The results of PCT test showed that the proportion of children in group A showed a increase trend,while that in group B was decreased with the increase of PCT concentrations. Conclusion PCT is an ideal indicator to distinguish atypical suppurative meningitis from viral encephalitis in children,and has the advantages of simple operation and higher sensitivity.
