摘　　要：目的通过评估影像学及临床结果,评价应用HyProcure距下关节稳定器治疗儿童柔韧性平足症的中期疗效。方法回顾性分析2013年6月—2016年8月应用HyProcure稳定器治疗的11例儿童柔韧性平足症(14足),其中男6例(8足),女5例(6足);左侧3例,右侧5例,双侧3例;年龄8~13岁,平均年龄(10.50±1.66)岁。对比术前、末次随访时,负重正位X线片距舟覆盖角(talonavicular coverage angle,TCA)、距骨第1跖骨角(talar 1st metatarsal angle,T1MT)、距骨第2跖骨角(talar 2th metatarsal angle,T2MT);侧位片距骨第1跖骨角(Meary角)、跟骨倾斜角(Pitch角)、距骨倾斜角(talar tilt angle,TT角);采用美国足踝外科协会(American Orthopedic Foot and Ankle Society,AOFAS)踝与后足功能评分和视觉模拟评分(visual analogue score,VAS)进行疗效评定。结果所有病例获得随访,平均随访52个月(30~81个月)。末次随访时TCA角、T1MT角、T2MT角、Meary角、Pitch角、TT角均较术前改善(P<0.05);VAS评分较术前显著降低,AOFAS评分较术前显著提高(P<0.05);优良率为92.86%。结论应用HyProcure稳定器治疗儿童柔韧性平足症,可以有效纠正畸形,外观及功能满意,中期疗效良好,远期疗效有待进一步随访。Objective To evaluate the mid-term effect of HyProcure talotarsal stabilization device in treatment of children's flexible flatfoot by radiographic outcomes and clinical results.Methods From June 2013 to August 2017,a total of 14 feet(11 children)were treated by HyProcure devices,including 6 boys(8 feet)and 5 girls(6 feet).3 cases were on the left,5 cases on the right and 3 cases on both sides.The average age is(10.50±1.66)years(range 8~13 years).To compare the data of pre-operative and last-time weightbearing X-rays,including the talonavicular coverage angle(TCA),talar 1st metatarsal angle(T1MT)and talar 2th metatarsal angle(T2MT),Meary angle,Pitch angle and Talar tilt angle(talar tilt angle,TT).The efficacy was evaluated by using the American Orthopedic Foot and Ankle Society(AOFAS)ankle and hindfoot score and visual analogue score(VAS)score.Results All children were followed up successfully for 30~81 months with the mean as 52 months.The angles of TCA,T1MT,T2MT,TT,Meary and Pitch angles were significantly improved(P<0.05)at the final follow-up visit.VAS score decreased,and AOFAS ankle and hindfoot score increased(P<0.05),compared with those before operation.The good rate was 92.86%.Conclusion The use of HyProcure talotarsal stabilization device in the treatment of children's flexible flatfoot can effectively correct deformities,satisfying appearance and function,and have good mid-term effects.The long-term effects need to be further studied.
