中药沐足与穴位按摩联合常规疗法治疗0级糖尿病足临床研究

摘　　要：目的:观察在常规疗法和护理措施基础上加用中药沐足与穴位按摩治疗0级糖尿病足的临床效果。方法:选择164例0级糖尿病患者作为研究对象,随机分成观察组、对照组各82例。2组均采用常规基础疗法和护理措施进行干预,观察组加用中药沐足联合穴位按摩进行治疗。2组患者均持续干预6个月。治疗前后测量踝肱指数、足部压力感觉阈值、足部震动感觉阈值,治疗后评定足部麻木评分与疼痛评分,比较2组患者的临床效果。结果:治疗后,观察组总有效率高于对照组,疗效优于对照组,差异均有统计学意义(P<0.01)。观察组踝肱指数、足部压力感觉阈值、足部震动感觉阈值均较治疗前升高(P<0.01),对照组上述指标治疗前后比较,差异均无统计学意义(P>0.05)。观察组踝肱指数、足部压力感觉阈值、足部震动感觉阈值均高于对照组(P<0.01)。观察组治疗6个月后的麻木、疼痛评分均低于对照组(P<0.01)。结论:在常规治疗和护理措施基础上加用中药沐足与穴位按摩治疗0级糖尿病足患者,可显著减轻患者足部的麻木感及疼痛感,改善压力觉与震动觉减退,对0级糖尿病足的发生发展有一定的二级预防作用。Objective:To observe the clinical effect of additional Chinese herbal foot bath and point massage based on routine therapy and nursing measures for grade 0 diabetic foot.Methods:Selected 164 cases of patients with grade 0 diabetic foot as study objects,and divided them into the observation group and the control group randomly,82 cases in each group.Both groups received routine basic therapy and nursing measures for intervention,and the observation group additionally received Chinese herbal foot bath combined with point massage.The intervention in both groups lasted for 6 months.Measured ankle-brachial index,foot pressure sensation threshold and foot vibration sensation threshold before and after treatment;evaluated the scores of numbness and pain in foot after treatment;compared the clinical effect between the two groups.Results:After treatment,the total effective rate in the observation group was higher than that in the control group,and the curative effect was better than that in the control group,differences being significant(P<0.01).The ankle-brachial index,foot pressure sensation threshold and foot vibration sensation threshold in the observation group were higher than those before treatment(P<0.01),and there was no significance in differences being found when comparing the indexes above in the control group before treatment with those after treatment(P>0.05).The ankle-brachial index,foot pressure sensation threshold and foot vibration sensation threshold in the observation group were higher than those in the control group(P<0.01).The scores of numbness and pain after six month of treatment in the observation group were lower than those in the control group(P<0.05).Conclusion:Based on routine treatment and nursing measures,the additional application of Chinese herbal foot bath and point massage in treating patients with grade 0 diabetic foot can significantly alleviate numbness and pain in foot,and improve the decline in pressure sensation and vibration sensation,which has certain effect of secondary prevent
