摘　　要：目的:分析多发骨结核18F-FDG与18F-FLT PET/CT的影像特征及诊断价值。方法:收集多发骨结核25例、骨转移瘤4例、多发骨髓瘤2例、良性骨病2例的资料,回顾性分析其病灶的18F-FDG及18F-FLT PET/CT影像特征,比较18F-FDG单独及联合18F-FLT PET/CT对多发骨结核的诊断价值。结果:25例多发骨结核患者共发现病灶117个,病灶对18F-FDG不同程度的高摄取,对18F-FLT低摄取;18F-FDG PET/CT骨及软组织感兴趣区的最大标准摄取值(SUVmax)、平均标准摄取值(SUVmean)及其靶非靶比值均高于18F-FLT PET/CT(P均<0.05)。18F-FDG PET/CT对多发骨结核诊断的灵敏度、特异度、准确度分别为32.0%(8/25)、25.0%(2/8)、30.3%(10/33);18F-FDG联合18F-FLT PET/CT分别为96.0%(24/25)、87.5%(7/8)、93.9%(31/33)。联合法诊断效能高于18F-FDG PET/CT(P均<0.05)。结论:18F-FDG联合18F-FLT双显像剂行PET/CT检查对多发骨结核的诊断具有重要价值。Aim:To analyze the imaging manifestations of 18F-FDG and 18F-FLT PET/CT of multiple bone tuberculosis,and its diagnostic value.Methods:The data of 25 cases of multiple bone tuberculosis,4 cases of bone metastases,2 cases of multiple myeloma and 2 cases of benign osteopathy were collected,and the 18F-FDG and 18F-FLT PET/CT imaging features of the lesions were retrospectively analyzed,the diagnostic efficacy of 18F-FDG and combined with 18F-FLT PET/CT for multiple bone tuberculosis were analyzed.Results:A total of 117 lesions were found in 25 patients with multiple bone tuberculosis,and the lesions showed high-uptake of 18F-FDG in varying degrees,low-uptake of 18F-FLT.The maximum standard uptake value(SUVmax),mean standard uptake value(SUVmean)and target-to-non-target ratio of 18F-FDG PET/CT in bone and soft tissue were all higher than those of 18F-FLT(all P<0.05).The sensitivity,specificity and accuracy of 18F-FDG PET/CT were 32.0%(8/25),25.0%(2/8)and 30.3%(10/33);and those of 18F-FDG combined with 18F-FLT PET/CT were 96.0%(24/25),87.5%(7/8)and 93.9%(31/33),respectively.The diagnostic efficacy of the combined method was higher than 18F-FDG PET/CT(all P<0.05).Conclusion:Combined use of 18F-FDG and 18F-FLT dual imaging agents for PET/CT examination is of great value in differential diagnosis of multiple bone tuberculosis.
