摘 要:目的比较空心钉内固定和人工股骨头置换术对老年移位性股骨颈骨折的治疗效果。方法随机选取在该院2006年7月—2018年8月期间治疗的100例老年移位性股骨颈骨折患者根据治疗方式差异分为对照组和实验组,对照组通过空心钉内固定手术治疗,实验组通过人工股骨头置换术治疗,比较两组患者的治疗效果。结果术中出血量,实验组(94.2±12.4)mL低于对照组(275.1±35.3)mL(t=34.189,P=0.000);手术时间,实验组(55.1±12.1)min少于对照组(97.2±33.6)min(t=8.336,P=0.000);住院天数,实验组(9.2±3.5)d短于对照组(13.7±4.5)d(t=5.582,P=0.000)。髋部骨折恢复,实验组(87.2±11.6)分优于对照组(81.2±10.7)分(t=2.688,P=0.008);生活质量,实验组(0.65±0.33)分优于对照组(0.55±0.13)分(t=1.994,P=0.049)。并发症发生率,实验组2%低于对照组14%(t=4.891,P=0.027)。结论人工股骨头置换术治疗老年移位型股骨颈骨折相比空心钉内固定术具有明显优势。Objective To compare the effects of hollow nail internal fixation and artificial femoral head replacement on elderly patients with displaced femoral neck fractures.Methods Randomly selected 100 elderly patients with displaced femoral neck fractures treated in the hospital from July 2006 to August 2018 were divided into the control group and the experimental group according to the difference in treatment methods.The control group was treated with hollow nail internal fixation surgery and the experimental group.Through artificial femoral head replacement surgery,the treatment effects of the two groups of patients were compared.Results The intraoperative blood loss was lower in the experimental group(94.2±12.4)mL than in the control group(275.1±35.3)mL(t=34.189,P=0.000);the operation time was less in the experimental group(55.1±12.1)min than in the control group(97.2±33.6)min(t=8.336,P=0.000);The length of hospital stay was shorter in the experimental group(9.2±3.5)d than in the control group(13.7±4.5)d(t=5.582,P=0.000).Hip fracture recovery,the experimental group(87.2±11.6)points were better than the control group(81.2±10.7)points(t=2.688,P=0.008);the quality of life,the experimental group(0.65±0.33)points were better than the control group(0.55±0.13)points(t=1.994,P=0.049).The complication rate was 2%lower in the experimental group than 14%in the control group(t=4.891,P=0.027).Conclusion Artificial femoral head replacement has obvious advantages compared with hollow nail internal fixation for elderly patients with displaced femoral neck fractures.
