摘　　要：目的:探讨Guidezilla延长导管在复杂冠状动脉(冠脉)病变介入治疗中的安全性和有效性。方法:回顾性分析泰达国际心血管病医院2018年1月-2019年6月经皮冠脉介入治疗(PCI)过程中使用Guidezilla延长导管的229例患者,整理相关数据,统计手术成功率和并发症发生率,评估Guidezilla延长导管的安全性和有效性。结果:229例患者冠脉病变AHA/ACC分型C型196例(85.6%),B型33例(14.4%),严重钙化121例(52.8%),血管重度扭曲152例(66.4%)。218例(95.2%)患者经桡动脉入路、11例(4.8%)患者经股动脉入路完成介入手术。使用最多的指引导管为EBU及SAL。9例重度钙化病变接受斑块旋磨术。PCI成功共计204例(89.1%),使用药物球囊处理2例,总成功率为90.0%。出现并发症21例(9.2%),无严重并发症,无死亡病例。结论:在复杂冠脉病变的PCI术中使用Guidezilla延长导管安全有效。Objective:To investigate the safety and efficacy of Guidezilla extension catheter in interventional therapy of complex coronary lesions.Method:A total of 229 patients who underwent percutaneous coronary intervention(PCI)with the Guidezilla extension catheter from January 2018 to June 2019 in TEDA International Cardiovascular Hospital were enrolled.Procedural and complications of all cases were analyzed.Result:Among the 229 patients,196(85.6%)were classified as type C,and 33(14.4%)as type B,121(52.8%)had severe calcification,and 152(66.4%)had severe vascular torsion.A total of 218(95.2%)patients underwent PCI via transradial pathway,while 11(4.8%)via transfemoral pathway.The most frequently used guiding catheters were EBU and SAL.Nine cases with severe calcification accepted rotational atherectomy.PCI was successful in 204 cases(89.1%),two cases were treated with drug eluting balloon;the total success rate was 90.0%.Complications occurred in 21 cases(9.2%);there was no serious complications and death.Conclusion:Guidezilla extension catheter is safe and effective in PCI of complex coronary lesions.
