音乐疗法缓解新生儿操作性疼痛有效性的系统评价

摘　　要：[目的]评价音乐疗法缓解新生儿操作性疼痛的有效性及安全性。[方法]检索the Cochrane Library、EMbase、PubMed、Web of Science、SIVAHL、中国知网、万方医学网、维普、中国生物医学文献数据库(CBM)中英文数据库中音乐疗法缓解新生儿操作性疼痛的随机对照试验(RCT),检索时限均为建库至2018年12月31日。由2名护理研究生按照纳入及排除标准进行文献筛选、文献评价及数据提取等工作,随后采用由the Cochrane Library提供的RevMan软件进行Meta分析或一般性描述。[结果]共纳入12篇RCT研究,共计样本量1363例。音乐疗法能够缓解操作性疼痛,且可降低操作过程中的心率变化幅度[MD=-8.39,95%CI(-11.49,-5.28),P<0.00001],但对于血氧饱和度的变化幅度[MD=0.61,95%CI(-1.42,2.64),P=0.56]无明显效果;在纳入的研究中无不良事件和副作用的报道。[结论]现有证据表明,音乐疗法与其他方法相比具有操作简单、成本低廉、副作用少等优点,可作为缓解新生儿操作性疼痛的方法。Objective:To evaluate the effectiveness and security of music therapy on relieving procedural pain in neonates.Methods:Randomized controlled trials(RCTs)on music therapy in relieving procedural pain were retrieved from the Cochrane Library,EMbase,PubMed,Web of Science,CINAHL,CNKI,WangFang Databases,VIP,and CBM.The retrieval time was from the inception to December 31,2018.according to the inclusion and exclusion criteria screening of literatures,evaluation of literatures and extraction of data were implemented by two nursing graduated students.RevMan software,which was provided by the Cochrane Library,was used for taking a Meta analysis or general description.Results:A total of twelve RCTs were included,involving 1363 neonates.The Meta-analysis results showed that music therapy could relieve procedural pain,reduce the heart rate change during operation[MD=-8.39,95%CI(-11.49,-5.28),P<0.00001]and respiratory rate.But there was no significant effect on the change range of blood oxygen saturation[MD=0.65,95%CI(-1.42,2.64),P=0.56].No adverse events or side effects were reported in the included literatures.Conclusion:Current evidence shows that,compared with other nonpharmacological methods,music therapy has the advantages in simple operation,low cost,less side effects,and so on.It can be used as a method to relieve the procedural pain in neonates.
