围产儿额面QRS电轴与出生胎龄的相关性研究

摘　　要：目的分析不同胎龄围产儿额面QRS电轴的变化规律及其与出生胎龄的相关性,为新生儿心电图诊断提供参考。方法回顾性分析不同出生胎龄的新生儿常规12导联心电图132例,将其根据出生胎龄分为A组(n=28,出生胎龄30~<33周)、B组(n=49,出生胎龄33~<37周)、C组(n=55,出生胎龄37~<42周)。观察三组围产儿的额面QRS电轴及其与出生胎龄的关系。结果三组不同性别围产儿的额面QRS电轴比较,差异无统计学意义(P>0.05)。三组围产儿的额面QRS电轴比较,差异具有统计学意义(P=0.000);C组围产儿的额面QRS电轴高于A组和B组,且B组围产儿的额面QRS电轴高于A组(P<0.001);随着出生胎龄的增加,新生儿额面QRS电轴右偏更明显。Pearson相关分析显示,围产儿额面QRS电轴与出生胎龄相关系数r=0.856(P=0.000);围产儿额面QRS电轴随出生胎龄的增加呈显著线性增长,线性回归方程为:y=7.212×出生胎龄-117.866(调整R^2=0.735)。结论围产儿额面QRS电轴随着出生胎龄的增加而右偏得更显著,电轴右偏与出生胎龄呈正相关。Objective To analyze the change regulation of frontal QRS axis in perinatal infants of different birth gestation ages and its correlation with birth gestation age,so as to provide reference for the diagnosis of neonatal electrocardiogram.Methods One hundred and thirty-two cases of normal routine 12-lead electrocardiogram in perinatal infants of different birth gestation ages were analyzed retrospectively.According to the birth gestation age,the infants were divided into group A(n=28,birth gestation age≥30 and<33 weeks),group B(n=49,birth gestation age≥33 and<37 weeks),and group C(n=55,birth gestation age≥37 and<42 weeks).The frontal QRS axis of the three groups and its correlation with birth gestation age were observed.Results There were no significant differences in frontal QRS axis of perinatal infants of different genders in the three groups(P>0.05).The difference of frontal QRS axis among the three groups was statistically significant(P=0.000);the frontal QRS axis of the group C was higher than that of the groups A and B,and that of the group B was higher than the group A(P<0.001);with the increase of birth gestation age,the right deviation of frontal QRS axis in neonates was more obvious.The Pearson correlation analysis showed that the correlation coefficient(r)between the frontal QRS axis and the birth gestation age was 0.856(P=0.000);the frontal QRS axis of perinatal infant increased linearly with the increase of birth gestation age,and the linear regression equation was y=7.212×gestational age-117.866(adjusted R^2=0.735).Conclusion The right deviation of frontal QRS axis is more significant with the increase of birth gestation age,and the right deviation of frontal QRS axis is positively correlated with birth gestation age.
