中性粒细胞和白蛋白检验在小儿严重脓毒症中的应用价值

摘　　要：目的分析中性粒细胞(NEU)和白蛋白(ALB)检验在小儿严重脓毒症中的应用价值。方法以本院2018年4月至2019年3月接收的60例脓毒症患儿作为观察对象,依据患儿病情严重程度将其分为严重组(18例,PCIS评分≤80分)和轻度组(42例,PCIS评分>80分)。检验并且比较两组患儿的临床相关指标,并分析脓毒症患儿病情严重程度的影响因素。结果严重组的NEU、BNP、PCT水平以及NAR均高于轻度组,ALB水平低于轻度组(P<0.05);两组的WBC、Hb、PLT、Lac、ALT、TBIL、BUN、CK-MB、APTT、CRP水平无显著差异(P>0.05)。NEU与ALB为预测患儿病情严重程度的独立性影响因素(P<0.05)。结论严重脓毒症患儿的ALB水平明显下降,NEU则明显提升,且随着病情进一步加重而发生改变,检验NEU与ALB水平对临床医务人员判断患儿疾病情况具有重要的参考价值。Objective To analyze the application value of the tests of neutrophils(NEU)and albumin(ALB)in children with severe sepsis.Methods Sixty children with sepsis enrolled in our hospital from April 2018 to March 2019 were seted as the study objests.According to the severity degree of the disease,the children were divided into severe group(18 cases,PCIS score≤80 points)and mild group(42 cases,PCIS score>80 points).The clinical related indexes of the two groups were examined and compared,and the influencing factors of the severity degree of sepsis were analyzed.Results The levels of NEU,BNP,PCT and NAR in the severe group were higher than those in the mild group,and the level of ALB was lower than that in the mild group(P<0.05).There were no significant differences in the WBC,Hb,PLT,Lac,ALT,TBIL,BUN,CK-MB,APTT and CRP level between the two groups(P>0.05).NEU and ALB were independent influencing factors to predict the severity degree of the disease in children(P<0.05).Conclusion The ALB level of children with severe sepsis decreased significantly,while the NEU level increased significantly,and changed with the further aggravation of the disease.Testing the NEU and ALB levels are of important reference value for clinical medical staff to judge the disease status of children.
