连续性血液净化治疗胆源性重症急性胰腺炎的临床效果及对患者免疫功能的影响

摘　　要：目的探究连续性血液净化(CBP)治疗胆源性重症急性胰腺炎(SAP)的临床效果及对患者免疫功能的影响。方法选取我院2018年1月至2019年1月收治的60例胆源性SAP患者,随机将其分为试验组(30例,常规治疗+CBP)和对照组(30例,常规治疗)。比较两组患者的治疗效果、免疫功能指标、血清生化指标。结果试验组的治疗总有效率为90.00%,明显高于对照组的63.33%(P<0.05)。治疗后,两组的CD4^+、CD4^+/CD8^+、HLA-DR均升高,且试验组明显高于对照组(P<0.05)。治疗后,两组的Cr、AMY、PCT水平均降低,且试验组明显低于对照组(P<0.05)。结论CBP治疗胆源性SAP的临床效果显著,能够抑制及清除炎性因子,维持机体内环境的稳定,进而使患者免疫功能得到提升,改善预后,值得临床推广应用。Objective To explore the clinical effect of continuous blood purification(CBP)in the treatment of biliary severe acute pancreatitis(SAP)and its influence on immune function of patients.Methods Sixty patients with biliary SAP in our hospital from January 2018 to January 2019 were selected and randomly divided into experimental group(30 cases,conventional treatment+CBP)and control group(30 cases,conventional treatment).The therapeutic effects,immune function indexes and serum biochemical indexes of the two groups were compared.Results The total effective rate of treatment in the experimental group was 90.00%,which was significantly higher than 63.33%of the control group(P<0.05).After treatment,the levels of CD4^+,CD4^+/CD8^+,HLA-DR of the two groups increased,and those of the experimental group were significantly higher than the control group(P<0.05).After treatment,the levels of Cr,AMY and PCT of the two groups decreased,and those of the experimental group were significantly lower than the control group(P<0.05).Conclusion CBP in the treatment of biliary SAP has a significant clinical effect,it can inhibit and remove inflammatory factors,maintain the stability of the internal environment,and improve the immune function and prognosis of patients,which is worthy of clinical promotion and application.
