摘　　要：目的探讨经皮肾镜下超声气压弹道碎石与输尿管镜下钬激光碎石治疗复杂性上尿路结石的临床效果。方法选取我院收治的150例复杂性上尿路结石的患者作为研究对象,根据手术方式的不同将其分为对照组和观察组,各75例。对照组给予输尿管镜下钬激光碎石术治疗,观察组给予经皮肾镜下超声气压弹道碎石术治疗。观察两组临床疗效。结果观察组手术时间、首次下床时间、住院时间均短于对照组,术中出血量少于对照组,结石清除率高于对照组(P<0.05)。术后,观察组hs-CRP、PCT、Scr、BUN水平及并发症总发生率均低于对照组(P<0.05)。观察组临床治疗总有效率和生活质量各项评分均高于对照组(P<0.05)。结论经皮肾镜下超声气压弹道碎石治疗复杂性上尿路结石更具优势,能有效优化手术指标,加快术后恢复,降低并发症发生率,提高结石清除率和hs-CRP、PCT、Scr、BUN水平,改善患者的生活质量。Objective To investigate the clinical effect of pneumatic ballistic lithotripsy under percutaneous nephroscope ultrasound and ureteroscopy holmium laser lithotripsy in the treatment of complex upper urinary calculi.Methods A total of 150 patients with complex upper urinary calculi admitted in our hospital were selected as the study objects.According to the different surgical methods,the patients were divided into control group and observation group,with 75 cases in each group.The control group was treated with ureteroscopy holmium laser lithotripsy,and the observation group was treated with pneumatic ballistic lithotripsy under percutaneous nephroscope ultrasound.The clinical effects of the two groups were observed.Results The operation time,first time of getting out of bed and hospitalization time of the observation group were shorter than those of the control group,the intraoperative blood loss was less than that of the control group,and the stone clearance rate was higher than that of the control group(P<0.05).After operation,the levels of hs-CRP,PCT,Scr and BUN and total incidence of complications in the observation group were lower than those in the control group(P<0.05).The clinical total effective rate of treatment and quality of life scores of the observation group were higher than those of the control group(P<0.05).Conclusion Pneumatic ballistic lithotripsy under percutaneous nephroscope ultrasound has more advantages in the treatment of complex upper urinary calculi.It can effectively optimize the operation indexes,accelerate the postoperative recovery,reduce the incidence of complications,improve the stone clearance rate and the levels of hs-CRP,PCT,Scr and BUN,and ameliorate the quality of life of patients.
