笑气吸入麻醉与咪达唑仑复合哌替啶静脉麻醉在胃肠镜检查中的效果及安全性

摘　　要：目的探讨笑气吸入麻醉与咪达唑仑复合哌替啶静脉麻醉在胃肠镜检查中的效果及安全性。方法选择2017年8月至2019年8月在我院接受胃肠镜检查的130例患者作为研究对象,依据随机数字表法将其分为A组(30例,利多卡因局麻)、B组(50例,咪达唑仑+哌替啶静脉麻醉)和C组(50例,笑气吸入麻醉与咪达唑仑复合哌替啶静脉麻醉)。比较三组的麻醉效果。结果B组、C组的麻醉优良率均高于A组(P<0.05)。B组、C组的不良反应总发生率低于A组,且C组的不良反应总发生率低于B组(P<0.05)。进镜后,A组的血压、心率显著高于进镜前及B组、C组(P<0.05)。三组检查耗时有显著差异,且C组短于A组、B组,B组短于A组(P<0.05)。B组、C组的满意度高于A组,且C组的满意度高于B组(P<0.05)。结论笑气吸入麻醉联合咪达唑仑、哌替啶静脉麻醉具有良好的麻醉效果,可提高胃肠镜检查的安全性,维持生命体征稳定,有利于缩短检查时间,提高患者满意度。Objective To investigate the effect and safety of nitrous oxide inhalation anesthesia combined with midazolam and pethidine intravenous anesthesia in gastrointestinal endoscopes.Methods A total of 130 patients undergoing gastrointestinal endoscopes in our hospital from August 2017 to August 2019 were selected as the study objects and randomly divided into group A(30 cases,lidocaine local anesthesia),group B(50 cases,midazolam+pethidine intravenous anesthesia)and group C(50 cases,nitrous oxide inhalation anesthesia combined with midazolam and pethidine intravenous anesthesia).The anesthetic effects in the three groups were compared.Results The excellent and good rate of anesthesia in the group B and group C was higher than that in the group A(P<0.05).The total incidence of adverse reactions in the group B and group C was lower than that in the group A,and the total incidence of adverse reactions in the group C was lower than that in the group B(P<0.05).After endoscopy,blood pressure and heart rate of the group A were significantly higher than those before endoscopy,in the group B and group C(P<0.05).There was significant difference in examination time among the three groups,that in the group C was shorter than the group A and group B,and that in the group B was shorter than the group A(P<0.05).The satisfaction of the group B and group C was higher than that of the group A,and the satisfaction of the group C was higher than that of the group B(P<0.05).Conclusion Nitrous oxide inhalation anesthesia combined with midazolam and pethidine intravenous anesthesia has a good anesthetic effect,it can improve the safety of gastrointestinal endoscopes,maintain the stability of vital signs,shorten the examination time,and ameliorate the satisfaction of patients.
