摘 要:目的探讨吲哚胺2,3-双加氧酶(IDO)在支气管哮喘(简称哮喘)发病机制中的作用及与IL-17水平的关系。方法将21只SPF级BALB/c小鼠随机分成对照组、布地奈德干预组和哮喘组,每组7只。尘螨提取液致敏、激发小鼠建立哮喘模型,HE染色观察气道炎症细胞的改变,取支气管肺泡灌洗液(BALF)行细胞学分析,ELISA法检测BALF中IL-17水平,免疫组化和Western blot法检测肺组织IDO蛋白表达。结果哮喘组肺组织可见明显的炎症细胞浸润、气道平滑肌细胞增生、管腔狭窄;干预组可见少量炎症细胞浸润,有气道平滑肌细胞的增生,管腔稍狭窄;对照组则无明显变化。哮喘组小鼠BALF中细胞总数、嗜酸性粒细胞数和淋巴细胞数均较对照组和干预组升高(P<0.05);哮喘组BALF中IL-17水平高于干预组和对照组(P<0.05);哮喘组肺组织IDO蛋白表达较对照组和干预组降低(P<0.05)。肺组织中IDO蛋白表达与BALF中IL-17水平呈负相关(r=-0.905,P<0.01)。结论肺组织IDO蛋白表达降低,导致免疫耐受缺陷和/或减弱,促进哮喘的发生、发展。IDO能抑制IL-17反应,减轻或抑制气道炎症。Objective To investigate the role of indoleamine 2,3-dioxygenase(IDO) expression in the pathogenesis of bronchial asthma and the relationship between IDO expression and IL-17 level.Methods Twenty-one SPF BALB/c mice were randomly divided into three groups with 7 mice each.Group C was taken as the blank control.After asthma models were established by sensitization of dust mite extracts,the mice in group A were treated with budesonide and those in group B were taken as the model control.HE staining was used to observe the changes of airway inflammatory cells.IL-17 level in bronchoalveolar lavage fluid(BALF) was detected by ELISA.Immunohistochemistry and Western blot were used to detect IDO protein expression in lung tissues.Results The lung tissues of group B showed visible obvious infiltration of inflammatory cells,airway smooth muscle cell hyperplasia,luminal stenosis,which of group A showed a small amount of inflammatory cell infiltration,proliferation of airway smooth muscle cells,slightly luminal narrower.There were no significant changes of lung tissues in group C.Compared to groups of A and C,the cell numbers of BALF in group B were more,which included the total cell number,eosinophilic eosinophile cell number and the lymphocyte cell number(P<0.05).The IL-17 level in BALF was higher in group B than that in groups of A and C(P<0.05).The protein expression of IDO in lung tissues was lower in group B than that in groups of A and C(P<0.05).The protein expression of IDO in the lung tissues was negatively correlated with IL-17 level in BALF(r=-0.905,P<0.01).Conclusion A decreased protein expression of IDO in lung tissues results in immune tolerance deficiency and/or reduction,which promotes the occurrence and development of bronchial asthma.IDO can inhibit IL-17 reaction,reduce or inhibit airway inflammation.
