超声引导下甲状腺结节粗针穿刺活检的临床应用及安全性评价

摘　　要：目的探讨甲状腺结节病例以超声引导下粗针穿刺活检术予以诊断的应用价值与安全性。方法回顾性分析我院2017年6月至2019年6月所收治甲状腺结节病患118例154个结节,均予以超声引导下甲状腺结节粗针穿刺活检,并将结果与手术病理检查结果对比,评价粗针穿刺活检的诊断准确率、特异性、敏感性以及阳性与阴性预测值,并观察结节直径与检查结果符合率的相关性。结果 154个结节成功穿刺151个(98.05%),穿刺后无1例穿刺并发症发生;穿刺结果恶性结节109个,良性42个,术后病理检查恶性结节107个,良性47个,UG-CNB诊断准确率94.81%(146/154)、敏感性98.13%(105/107)、特异性91.11%(41/45)、阳性预测值96.33%(105/109)、阴性预测值97.62%(41/42);结节直径与UG-CNB诊断符合率无相关性。结论基于超声引导下所行甲状腺结节粗针穿刺活检具有较高的特异性与敏感性,可切实提高甲状腺恶性结节诊断率,为安全可行的术前确诊方法,具临床推广及借鉴价值。Objective To investigate the diagnostic value and safety of ultrasound-guided coarse needle biopsy for thyroid nodules. Methods A retrospective analysis of our hospital from June 2017 to June 2019 by the 154 patients, 118 cases of thyroid nodule were nodules, are to be guided by ultrasound of thyroid nodule coarse needle biopsy, and the results compared with surgical pathologic examination results and evaluation of coarse needle biopsy diagnosis accuracy, specificity, sensitivity and positive and negative predictive value, and watch the nodule diameter and the coincidence of the results of the tests. Results 151 of 154 nodules were successfully punctured(98.05%). The diagnostic accuracy of ug-cnb was 94.81%(146/154), the sensitivity was 98.13%(105/107), the specificity was 91.11%(41/45), the positive predictive value was 96.33%(105/109), and the negative predictive value was 97.62%(41/42). Nodule diameter was not correlated with the diagnostic coincidence rate of ug-cnb. Conclusion Ultrasound-guided coarse needle biopsy of thyroid nodules has high specificity and sensitivity, which can effectively improve the diagnosis rate of malignant thyroid nodules. It is a safe and feasible preoperative diagnosis method and has clinical promotion and reference value.
