冠心病病人血浆白细胞介素21表达及其临床意义

摘　　要：目的探讨冠心病病人血浆白细胞介素-21(IL-21)表达及其临床意义。方法选取2016年11月—2017年10月我院收治的冠心病病人131例作为冠心病组,病人血管狭窄程度>50%;同期冠状动脉造影未见明显狭窄病变者30例为对照组。将冠心病病人根据不同疾病类型分为稳定型心绞痛组、不稳定型心绞痛组和心肌梗死组;根据冠状动脉病变支数分为单支病变组、双支病变组、三支病变组。采用酶联免疫吸附法(ELISIA)检测病人血浆IL-21浓度,分析IL-21与冠状动脉病变程度的关系。结果与对照组相比,冠心病组血浆IL-21浓度明显增加(P<0.01);与稳定型心绞痛组相比,不稳定型心绞痛组和心肌梗死组IL-21血浆浓度升高,差异均有统计学意义(P<0.01);与单支病变组比较,双支病变组及三支病变组血浆IL-21水平均明显增加(P<0.01),但双支病变组和三支病变组IL-21浓度比较差异无统计学意义(P>0.05)。结论血浆IL-21水平在冠心病病人中表达升高,可作为预测冠心病严重程度的有效指标。Objective To study the expression and clinical significance of plasma IL-21 in coronary heart disease(CHD).Methods One hundred and thirty-one patients with CHD were selected as CHD group,the vascular stenosis of patients was more than 50%,and 30 patients with out obvious vascular stenosis on coronary arteriography were selected as control group.According to disease types,patients with CHD were divided into stable angina group,unstable angina group,and myocardial infarction(MI)group.According to the number of lesioned coronary arteries,patients with CHD were divided into single-vessel disease group,double-vessel disease group,and three-vesseI disease group.The plasma IL-21 concentration was detectd by ELISIA method,and the correlation between IL-21 and coronary lesions was analyzed.Results Compared with control group,the plasma IL-21 concentration in patients with CHD was increased significantly(P<0.01).Compared with stable angina group,IL-21 levels in unstable angina group and MI group were further increased(P<0.01).Compared with single-vessel disease group,plasma IL-21 levels in patients with double-vessel disease group and three-vessel disease group were significantly increased(P<0.01),but there was no significant difference between double-vessel disease group and three-vesseI disease group(P>0.05).Conclusion Plasma IL-21 level is elevated in patients with CHD,which may be a predictor of predicting the severity of CHD.
