摘 要:目的探讨外固定架联合锁定加压钢板内固定治疗C型Pilon骨折的临床疗效。方法选取2013年12月至2017年12月东南大学附属中大医院江北院区收治的102例C型Pilon骨折患者,采用随机数字表法分为观察组和对照组,每组51例。观察组采用外固定架联合锁定加压钢板内固定治疗,对照组采用锁定加压钢板内固定治疗。比较两组患者的术后住院时间、骨折愈合时间及术后并发症发生情况,采用Burwell Chamley评分评价两组患者的骨折复位情况,采用Tornetta评分评价两组患者的踝关节功能恢复情况。结果观察组患者手术时间、术后住院时间及骨折愈合时间均明显短于对照组,术中出血量明显低于对照组,差异有显著性(P<0.05)。两组骨折复位情况比较差异无显著性(P>0.05)。观察组患者踝关节功能恢复情况明显优于对照组,术后并发症发生率明显低于对照组,差异均有显著性(P<0.05)。结论外固定架联合锁定加压钢板内固定治疗C型Pilon骨折可促进患者踝关节功能恢复,缩短骨折愈合时间,术后并发症少,值得临床应用。Objective To explore the effect of external fixator combined with locking compression plate(LCP) in the treatment of type C Pilon fractures. Method The 102 patients with type C distal radius fractures who underwent surgery, according to different operation methods, were divided into LCP internal fixation combined with external fixator group(observation group, 51 patients) and single LCP internal fixation group(control group, 51 patients). The length of hospital stay, fracture healing time, fracture reduction, ankle score and complications were compared between the two groups. Result The operation time, intraoperative blood loss, postoperative hospitalization time and fracture healing time of the observation group were significantly shorter than those of the control group(P<0.05). There was no significant difference in fracture reduction between the two groups(P>0.05). The recovery of ankle joint function in the observation group was significantly better than that in the control group, and the incidence of postoperative complications was significantly lower than that in the control group, with significant differences(P<0.05). Conclusion External fixator combined with locking compression plate internal fixation in the treatment of C-type Pilon fracture is conducive to promoting the recovery of ankle function, shortening the healing time of fracture, and less postoperative complications.
