摘　　要：目的研究沙美特罗/丙酸氟替卡松联合噻托溴铵治疗老年慢性阻塞性肺疾病(COPD)的效果及其对患者肺功能、动脉血气指标和血清炎症因子的影响。方法选择太原钢铁(集团)有限公司总医院2016年1月至2019年2月诊治的600例老年COPD患者作为研究对象,根据其治疗方法分为对照组(250例)和联合组(350例),对照组给予沙美特罗/丙酸氟替卡松治疗,联合组在对照组基础上联合噻托溴铵治疗。两组均连续治疗3个月,比较两组患者治疗前后肺功能、动脉血气指标和血清炎症因子水平变化,并评估其疗效。结果治疗后,两组患者肺功能指标水平、动脉血氧饱和度(SaO2)、动脉血氧分压(PaO2)水平均上升,血清炎症因子水平和动脉血二氧化碳分压(PaCO2)水平均降低,且联合组上升或降低幅度高于对照组(P<0.05);联合组总有效率(94.85%,332/350)高于对照组(90.00%,225/250),差异有统计学意义(P<0.05)。结论沙美特罗/丙酸氟替卡松联合噻托溴铵应用于老年COPD患者,可明显改善其肺功能以及动脉血气指标,减轻炎症反应,从而提高治疗效果。Objective To study the therapeutic effects of salmeterol/fluticasone propionate(FP)combined with tiotropium,and its influence on pulmonary function,arterial blood gas indexes and serum inflammatory factors in the treatment of elderly patients with chronic obstructive pulmonary disease(COPD).Methods A total of 600 elderly patients with COPD who were treated in the General Hospital of Taiyuan Iron and Steel Group from January 2016 to February 2019 were enrolled as study objects.According to treatment methods,they were divided into control group(250 cases)and combination group(350 cases).The control group was treated with salmeterol/FP,while the combination group was treated with tiotropium and salmeteroly/FP.Both groups were continuously treated for 3 months.The changes in pulmonary function,arterial blood gas indexes and levels of serum inflammatory factors before and after treatment were compared between the two groups.And curative effect was evaluated.Results After treatment,the levels of pulmonary function indexes,arterial oxygen saturation(SaO2)and arterial oxygen partial pressure(PaO2)in both groups significantly increased,while the levels of serum inflammatory factors and arterial partial pressure of carbon dioxide(PaCO2)significantly decreased,the increase and decrease in combination group were more significant than those in control group(P<0.05).The total response rate of combination group(94.85%,332/350)was significantly higher than that of control group(90.00%,225/250),P<0.05.Conclusions The application of salmeterol/FP combined with tiotropium for elderly COPD patients can significantly improve pulmonary function and arterial blood gas indexes,alleviate inflammatory reactions,thus improving curative effect.
