复春散Ⅱ号、凉血解毒方联合重组人生长激素对重度烧伤患者血清促炎因子TNF-α、CRP、IL-6、蛋白质代谢及免疫功能的影响

摘　　要：目的分析复春散Ⅱ号、凉血解毒方联合重组人生长激素对重度烧伤(SDB)患者血清促炎因子TNF-α、CRP、IL-6、蛋白质代谢及免疫功能的影响。方法将医院2017年3月—2019年3月收治的SDB患者129例按随机数字表法(RTNM)将其分成A、B组,各65例、64例,其中A组单纯予以复春散Ⅱ号、凉血解毒方治疗,B组则在A组治疗的基础上联合重组人生长激素治疗,将两组各项数据纳入SPSS 21.0软件处理,比较两组治疗前后的血清促炎因子TNF-α、CRP、IL-6表达及蛋白质代谢及免疫功能。结果1)A、B组治疗前的血清TNF-α、IL-6、CRP表达水平相比无显著差异(P>0.05),B组治疗后血清TNF-α、IL-6、CRP表达水平均显著低于A组(P<0.05)。2)A、B组治疗前的免疫球蛋白(IgG、IgA、IgM)和T细胞亚群(CD4、CD8、CD4/CD8)相比无显著差异(P>0.05),B组治疗后以上几项指标均高于A组(P<0.05)。3)A、B组治疗前氮平衡、白蛋白、前白蛋白及转铁蛋白水平相比均无显著差异(P>0.05),B组治疗后以上指标均高于A组(P<0.05)。4)经ROC曲线分析发现,TNF-α、IL-6、CRP、蛋白质代谢及免疫功能在SDB患者的创面愈合中均有较好的临床评估价值。结论在SDB患者临床治疗中科学、合理的予以复春散Ⅱ号、凉血解毒方联合重组人生长激素能显著降低重度烧伤患者机体炎症介质的水平,改善蛋白质代谢和免疫功能。可选择性地将以上几项指标视作患者创面愈合情况的评价标准之一,临床应用价值较好。Objective To analyze the effect of Fuchun PowderⅡand Liangxue Jiedu Formula and recombinant human growth hormone on serum pro-inflammatory factors TNF-α,CRP,IL-6,protein Metabolism and immune function in patients with severe burn(SDB).Methods A total of 129 patients with SDB admitted to our hospital from March 2017 to March 2019 were divided into group A(n=65)and group B(n=64)according to RTNM.Group A was treated with Fuchun PowderⅡand Liangxue Jiedu Formula only.Group B was treated with recombinant human growth hormone on the basis of group A,and the data of the two groups was included in SPSS 21.0 software to compare the treatment of the two groups.The expressions of TNF-α,CRP,IL-6,protein Metabolism and immune function before and after treatment were compared.Result 1)There was no significant difference in the levels of TNF-α,IL-6 or CRP between group A and group B before treatment(P>0.05).2)There was no significant difference in immunoglobulin(IgG,IgA,IgM)and T cell subsets(CD4,CD8,CD4/CD8)between group A and group B before treatment(P>0.05).3)There was no significant difference in levels of nitrogen balance,albumin,prealbumin or transferrin between group A and group B before treatment(P>0.05),and the above indexes in group B after treatment were higher than those in group A(P<0.05).4)ROC curve analysis showed that TNF-α,IL-6,CRP,protein Metabolism and immune function had better clinical evaluation value in wound healing of SDB patients.Conclusion In the clinical treatment of SDB patients,Fuchun Powder II and Liangxue Jiedu Formula combined with recombinant human growth hormone can significantly reduce the levels of inflammatory mediators in patients with severe burns,and the above indicators can be selected as one of the evaluation criteria of wound healing,with good clinical application value.
