中药鼻腔冲洗联合布地奈德混悬液对鼻息肉术后康复和血清细胞因子的影响

摘　　要：目的探讨中药鼻腔冲洗联合布地奈德混悬液冲洗对鼻息肉术后康复和血清细胞因子的影响。方法选取2018年5月—2019年1月行鼻内镜手术的鼻息肉患者150例,按照随机数字表法随机分成对照组和观察组,每组75例。对照组患者术后予以布地奈德混悬液经鼻雾化吸入治疗,而对观察组患者在予以布地奈德混悬液治疗的基础上联合予以鼻窦炎口服液鼻腔冲洗治疗。治疗后,分别对两组患者的症状改善情况、血清细胞因子水平以及生活质量进行比较。结果①观察组患者的各主观症状体征VAS得分均显著低于对照组患者,差异有统计学意义(P<0.05);②观察组患者的鼻内镜黏膜形态评分(Lund-Kenenedy)、鼻窦CT评分(Lund-Mackey)均显著优于对照组患者,差异有统计学意义(P<0.05);③观察组患者的血清肿瘤坏死因子-α(TNF-α)、干扰素-γ(IFN-γ)、白细胞介素-8(IL-8)以及白细胞介素-17(IL-17)水平均显著低于对照组患者,差异有统计学意义(P<0.05);④观察组患者的以及鼻窦炎性疾病特异性生存质量量表(SNOT-20)评分显著高于对照组患者,差异有统计学意义(P<0.05)。结论采用中药鼻腔冲洗联合布地奈德混悬液对行鼻内镜下鼻息肉切除术后患者进行治疗,能够有效改善患者临床症状,降低血清炎症因子水平,促进鼻黏膜的术后恢复,具有显著的临床治疗效果。Objective To explore the effect of Chinese medicine nasal irrigation combined with budesonide suspension irrigation on the recovery and serum cytokines of nasal polyps after operation.Methods A total of 150 patients with nasal polyps who underwent endoscopic sinus surgery from May 2018 to January 2019 were randomly divided into control group and observation group,75 cases in each group.The patients in the control group were treated with budesonide suspension after nasal atomization inhalation,while those in the observation group were treated with budesonide suspension combined with nasal irrigation with nasosinusitis oral liquid.After intervention,the improvement of symptoms,serum levels of cytokine and quality of life were compared between the two groups.Results①The VAS scores of subjective symptoms and signs in the observation group were lower than those of the control group(P<0.05).②The Lund-Kenenedy and Lund-Mackey scores of the observation group were significantly better than those of the control group(P<0.05).③The serum levels of TNF-α,IFN-γ,IL-8 and IL-17 in the observation group were lower than those of the control group(P<0.05).④The scores and the Sinusitis Disease Specific Quality of Life Scale(SNOT-20)of the observation group were significantly higher than those of the control group(P<0.05).Conclusion Chinese medicine nasal irrigation combined with budesonide suspension can effectively improve the clinical symptoms of patients after nasal endoscopic polypectomy,reduce the levels of serum inflammatory factors,and promote the recovery of nasal mucosa after surgery.It has significant clinical effect.
