多种自身抗体联合检测诊断系统性红斑狼疮的价值

摘　　要：目的分析多种自身抗体联合检测诊断系统性红斑狼疮的应用意义。方法抽取郑州大学人民医院2015年1月至2017年12月收治的系统性红斑狼疮患者96例为观察组,同期选择健康体检者96例为健康对照组。回顾性分析两组全部的临床资料。两组均实施多种自身抗体联合检测,观察分析两组检测结果。结果观察组AHA、抗SSB、抗SSA、抗nRNP、ARPA、抗Sm、AunA、抗双链DNA抗体(抗dsDNA)、ANA等指标阳性率高于对照组,差异有统计学意义(P<0.05);观察组活动期抗Sm、AunA阳性率高于非活动期,但差异未见统计学意义(P>0.05),而抗dsDNA高于非活动期,差异有统计学意义(P<0.05)。抗Sm+AunA与AunA+抗dsDNA联合检测阳性率高于抗dsDNA单独检测,差异有统计学意义(P<0.05);而抗Sm+抗dsDNA与抗Sm+AunA+抗dsDNA联合检测阳性率高于抗体单独检测,差异有统计学意义(P<0.05)。结论在临床诊断系统性红斑狼疮患者时,采取多种自身抗体联合检测可提高诊断效率、特异性、敏感度,其值得临床应用推广。Objective To analysis of the significance of multiple autoantibodies in the diagnosis of systemic lupus erythematosus(SLE).Methods A total of 96 SLE patients(SLE group)and 96 healthy controls(healthy control group)were selected who visited People’s Hospital of Zhengzhou University from January 2015 to December 2017.The results of the two groups were observed and analyzed.Results The positive rates of Aha,anti SSB,anti SSA,anti nrnp,ARPA,anti SM,auna,anti dsDNA antibody(anti dsDNA)and ANA in the observation group were higher than those in the control group(P<0.05);the positive rate of anti SM and auna in the active phase was higher than that in the inactive phase,but the difference was not statistically significant(P>0.05),while the anti dsDNA was higher than that in the inactive phase,the difference was statistically significant(P<0.05).The positive rate of combined detection of anti SM+auna and auna+anti dsDNA was higher than that of single detection of anti dsDNA(P<0.05);the positive rate of anti SM+anti dsDNA combined with anti SM+auna+anti dsDNA was higher than that of antibody alone(P<0.05).Conclusions In the clinical diagnosis of systemic lupus erythematosus,the combination of multiple autoantibodies can improve the efficiency,specificity and sensitivity of diagnosis,which is worthy of clinical application and promotion.
