摘　　要：目的观察改良鼻丘径路额窦开放术治疗慢性额窦炎的效果及对患者嗅觉功能和术后复发的影响。方法回顾性分析2017年1月至2019年1月武装警察部队河南省总队医院收治的80例慢性额窦炎患者的资料,其中48例实施鼻内窥镜下改良鼻丘径路额窦开放术为观察组,32例实施鼻内窥镜下常规入路额窦开放术为对照组。比较两组患者围术期指标,临床疗效,手术前、手术2周后嗅觉功能,以及术后6个月内复发情况。结果观察组患者手术时间、术中出血量少于对照组(P<0.05);观察组患者手术总有效率略高于对照组,差异未见统计学意义(P>0.05);术后2周,两组患者平均嗅觉阈值评分均较术前降低,且观察组低于对照组(P<0.05);术后随访6个月,观察组患者复发率低于对照组(P<0.05)。结论改良鼻丘径路额窦开放术治疗慢性额窦炎可减少手术创伤,降低复发率,利于患者嗅觉功能恢复。Objective To observe the effects of modified nasal hill approach frontal sinus opening surgery on patients with chronic frontal sinusitis.Methods A retrospective analysis was performed on the data of 80 patients with chronic frontal sinusitis admitted to Henan Provincial General Hospital of the Chinese People’s Armed Police Force from January 2017 to January 2019.Among them,the 48 patients with modified modified nasal hill approach frontal sinus opening surgery under nasal endoscope were included in inobservation group,and 32 patients with routineapproach frontal sinus opening surgery under nasal endoscope were included in control group.Perioperative indexes,surgical clinical efficacy,olfactory function before surgery and after 2 weeks of surgeryand recurrence within 6 months after surgery were compared between the two groups.Results The operative time and intraoperative blood lossin observation group were significantly shorter than those in control group(P<0.05).The total effective rate of surgery in observation group was slightly higher than that in control group(P>0.05).Two weeks after surgery,the average olfactory threshold scores in the two groups were decreased compared with those before surgery,and the score in observation group was lower than that in control group(aP<0.05).Six months of follow-up after surgery,the recurrence rate in observation group was lower than that in control group(P<0.05).Conclusions Modifiednasal hill approach frontal sinus opening surgery for chronic frontal sinusitis can reduce surgical trauma,lower recurrence rate,and facilitate the recovery of olfactory function.
