40例ABO血型抗原减弱的血清学表现及原因分析

摘　　要：目的探究ABO血型抗原减弱的血清学表现和原因。方法随机选取2018年1月至2019年12月我院收治的ABO血型抗原减弱患者40例作为观察对象,分析总结患者抗原减弱的原因及血清学表现。结果在选取的40例患者中,检测出A型抗原减弱11例,B型抗原减弱14例,AB型A抗原减弱7例,AB型B抗原减弱8例,37例患者有明确的临床诊断,3例无法明确诊断,无法排除血型变异是否是疾病引起的,其中H抗原增强出现31例,有9例没有出现H抗原增强的情况。结论鉴定ABO血型的过程中,血清学有很重要的作用,但是血清学还存在一些不足,无法区分疑难血型,医疗技术在不断发展,分子生物学在进步,可以利用基因分型的方式判定ABO疑难血型,这种技术在未来会被广泛应用于临床使用,为临床患者个体化血液管理提供基础,为输血安全提供保障。Objective To explore the serological manifestations and causes of ABO blood group antigen attenuation.Methods 40 patients with ABO blood group antigen attenuation admitted to our hospital from January 2018 to December 2019 were randomly selected as observation objects,and the causes and serological manifestations of the patients’antigen attenuation were analyzed and summarized.Results Among the 40 patients selected,11 cases of type A antigen attenuation,14 cases of type B antigen attenuation,7 cases of type AB antigen attenuation,8 cases of type AB antigen attenuation,37 patients had definite clinical diagnosis,3 cases could not be clearly diagnosed,blood type variation caused by disease could not be excluded,31 patients had H antigen enhancement,and 9 cases did not have H antigen enhancement.Conclusion Serology plays an important role in ABO blood type identification.However,there are still some deficiencies in serology when distinguishing some difficult blood types.With the continuous development of science and technology,molecular biology has been improved.Genotyping method to determine ABO difficult blood types will be widely used in clinical application in the future,providing a strong guarantee for realizing individualized blood management of clinical patients and ensuring blood transfusion safety.
