比较全身麻醉和椎管内麻醉在全髋关节置换术中的临床应用价值

摘　　要：目的研究全身麻醉以及椎管内麻醉方案应用在全髋关节置换术当中的价值。方法本文以分组对照方法进行研究,所选择的患者在2017年4月至2018年6月来我院进行全髋关节置换术治疗,本文选择82例作为研究对象,并同时以随机方法分为观察组和对照组,每组平均41例。为对照组患者通过全身麻醉方案进行麻醉,观察组患者采用椎管内麻醉方案进行麻醉,对两组不同麻醉方式所取得的麻醉质量和效果进行评比。结果观察组麻醉优良38例,麻醉优良率为92.68%,对照组麻醉优良31例,麻醉优良率为75.61,观察组麻醉优良率高于对照组,P<0.05;观察组不良反应发生5例,占12.20%,对照组不良反应12例,占29.27%,观察组低于对照组,P<0.05;观察组麻醉满意40例,占97.56%,对照组麻醉满意33例,占80.49%,观察组高于对照组,P<0.05;观察组的收缩压、舒张压和心率分别为(101.3±10.5)mm Hg、(78.4±6.3)mm Hg和(66.7±4.3)次/分,对照组分别为(128.4±18.2)mm Hg、(96.4±8.2)mm Hg和(88.5±6.2)次/分,P<0.05。结论对全髋关节置换术患者采用椎管内麻醉方案进行麻醉所取得的效果明显优于全身麻醉方案,可有效提升患者麻醉质量,降低不良反应发生率促使患者,提高患者的满意度,平稳患者的血压和心率,减少麻醉室滞留时间,具有推广应用的价值。Objective To study the value of general anesthesia and intraspinal anesthesia in total hip replacement.Methods This study was conducted by grouping control method.The selected patients were admitted to our hospital for total hip arthroplasty from April 2017 to June 2018.82 patients were selected as subjects in this study,and were randomly divided into observation group and control group,with an average of 41 patients in each group.Patients in the control group were anesthetized by general anesthesia program,and patients in the observation group were anesthetized by intraspinal anesthesia program.The quality and effect of anesthesia obtained by different anesthesia methods in the two groups were compared.Results 38 patients in the observation group had excellent anesthesia,and the excellent and good rate of anesthesia was 92.68%;31 patients in the control group had excellent anesthesia,and the excellent and good rate of anesthesia was 75.61.The excellent and good rate of anesthesia in the observation group was higher than that in the control group(P<0.05).Adverse reactions occurred in 5 cases(12.20%)in the observation group,and in 12 cases(29.27%)in the control group.Adverse reactions in the observation group were lower than those in the control group(P<0.05).40 patients in the observation group were satisfied with anesthesia,accounting for 97.56%,and 33 patients in the control group were satisfied with anesthesia,accounting for 80.49%.The observation group was higher than the control group(P<0.05).The systolic blood pressure,diastolic blood pressure and heart rate of the observation group were(101.3±10.5)mm Hg,(78.4±6.3)mm Hg and(66.7±4.3)times/min,respectively.The control group was(128.4±18.2)mm Hg,(96.4±8.2)mm Hg and(88.5±6.2)times/min,respectively,P<0.05.Conclusion For the spinal canal anesthesia methods in patients with total hip replacement for anesthesia is obviously better than the effect of general anesthesia,can effectively improve the quality of anesthesia,patients with lower incidence of adverse
