老年正常高值血压人群的静息心率与颈股动脉脉搏波传导速度的关系

摘　　要：目的探讨正常高值血压人群的静息心率与颈股动脉脉搏波传导速度(cfPWV)的关系。方法入选2017年3月至2019年3月在福建医科大学附属第一医院门诊就诊的正常高值血压者288人,选择同期在门诊就诊的正常血压者326人作为对照组。所有受试者测量身高、体质量、血压和静息心率,测定血生化指标、cfPWV。结果与对照组相比,正常高值血压组的cfPWV明显增高[(8.60±1.38)比(7.79±1.24)m/s,P<0.01]。无论是在正常高值血压组还是在对照组中,cfPWV均随着年龄的增长而增高,男性的cfPWV明显高于女性(均P<0.01)。在正常高值血压组中,控制年龄、性别后的偏相关分析提示,静息心率与cfPWV呈正相关(r=0.230,P<0.01)。多元线性逐步回归分析显示,校正其他心血管病危险因素后,静息心率、年龄、性别都是正常高值血压人群cfPWV的独立相关因素(均P<0.01);分层分析结果显示,在年龄≥60岁正常高值血压人群中,无论男性还是女性,静息心率都与cfPWV独立相关(均P<0.01);而在年龄<60岁正常高值血压人群中,静息心率与cfPWV无独立相关。结论在老年正常高值血压人群中,静息心率与cfPWV独立相关。Objective To investigate the relationship between resting heart rate and carotid-femoral pulse wave velocity(cfPWV)in patients with prehypertension.Methods A total of 288 outpatients with prehypertension and 326 normotensive outpatients from the First Affiliated Hospital of Fujian Medical University were enrolled between March 2017 and March 2019.Three hundred and twenty-six normotensive outpatients were served as controls.Body height and weight,resting heart rate and blood pressure of all patients were recorded.Blood biochemical indexes and cfPWV were determined.Results Compared with the control group,cfPWV in the prehypertension group was significantly higher[(8.60±1.38)vs(7.79±1.24)m/s,P<0.01].There was an increase in cfPWV with advancing age both in the prehypertension group and in the control group,and cfPWV in male was higher than in female.In the prehypertension group,after stratified with age and gender,partial correlation analysis suggested that resting heart rate was positively correlated with cfPWV(r=0.230,P<0.01).After adjusted for other cardiovascular risk factors,multivariable stepwise regression analysis indicated that resting heart rate,age and gender were independently associated with cfPWV in the prehypertension group(all P<0.01).Moreover,whether in male or in female,resting heart rate was independently associated with cfPWV in the patients with prehypertension aged≥60 years(all P<0.01).However,in the patients with prehypertension aged<60 years,no correlation was observed between resting heart rate and cfPWV.Conclusion In the elderly with prehypertension,resting heart rate is independently associated with cfPWV.
