刘德山运用膏方治疗癌病用药规律分析

摘　　要：目的:基于中医传承辅助系统软件对刘德山教授近年来运用膏方治疗癌病的用药组方规律进行分析。方法:收集2013年7月至2018年7月刘德山教授于山东大学齐鲁医院中医科门诊诊疗的癌病病例中治疗应用膏方者,建立膏方数据库并逐一录入中医传承辅助系统(V2.5)软件,运用规则分析、改进的互信息法、复杂系统熵聚类等数据挖掘方法,分析运用膏方治疗癌病的组方规律。结果:经筛选,46首膏方中共计出现136味中药,使用频次前10位的中药分别为黄芪、白术、茯苓、党参、女贞子、薏苡仁、白花蛇舌草、炒麦芽、砂仁、山药,常用药对为黄芪-白术、白术-茯苓、党参-黄芪等,发现核心药物组合9对,挖掘出新药队共9个。结论:刘德山教授运用膏方治疗癌病疗效显著,经分析遣方用药时注重扶正补虚,防病治病,动静结合,通补兼施,补益兼顾祛邪,祛邪不忘顾护脾胃功能,力求达到阴平阳秘、以衡为期的目的。Objective:To analyze the medication and prescription regularity in Professor LIU Deshan’s application of paste prescription for cancer treatment in recent years based on the“Chinese medicine inheritance assistance system software”.Methods:Cancer cases diagnosed and treated by Professor LIU Deshan with paste prescriptions in the outpatient of traditional Chinese medicine(TCM)department of Qilu Hospital from July 2013 to July 2018 were collected.A paste prescription database was established and input into the Chinese medicine inheritance assistant system(V2.5).Using data mining methods such as rule analysis,improved mutual information method and complex system entropy clustering to analyze medication regularity in Professor LIU Deshan’s use of paste prescription to treat cancer.Results:A total of 136 herbal medicines were found in 46 paste prescriptions.The top 10 herbal medicines which were used frequently were Huangqi(Astragali Radix),Baizhu(Atractylodis Macrocephalae Rhizoma),Fuling(Poria),Dangshen(Codonopsis Radix),Nvzhenzi(Ligustri Lucidi Frucitus),Yiyiren(Coicis Semen),Baihuasheshecao(Oldenlandia Diffusa),Chaomaiya(Hordei Fructus Germinatus),Sharen(Fructus Amomi),Shanyao(Dioscoreae Rhizoma).The commonly used medicine pairs were Huangqi-Baizhu,Baizhu-Fuling,Dangshen-Huangqi and so on.A total of 9 core medicine combinations were found,and 9 new medicine teams were excavated.Conclusions:Professor LIU Deshan’s paste prescriptions have significant curative effect in the treatment of cancer.According to the analysis,he pays attention to reinforcing healthy qi,preventing and curing the disease,combining the dynamics and statics,applying both unblocking and benefiting method.He also attaches importance on both benefiting the healthy qi and removing the pathogenic qi,as well as taking care of the function of the spleen and stomach,so as to reach the balance between yin and yang.
