摘 要:目的探讨血清Catestatin、Salusin-α水平与妊娠期高血压疾病患者左心室功能下降的相关性。方法选取2016年5月至2018年8月收治的妊娠期高血压患者216例为研究对象。分为妊娠期高血压组(n=84),轻度子痫前期组(n=75),重度子痫前期组(n=57);选取同期健康孕妇100人作为对照组。比较各组间基本资料、左心室舒张功能、血清Catestatin、Salusin-α水平,并进行Pearson相关性和Logistic回归分析。结果重度子痫前期组体质量指数(BMI)、收缩压、舒张压高于轻度子痫前期组、妊娠期高血压组及对照组(P<0.05)。重度子痫前期组二尖瓣前向血流A峰、A峰的速度时间积分(AVTI)高于轻度子痫前期组、妊娠期高血压组及对照组,二尖瓣前向血流E峰、E峰的速度时间积分(EVTI)、E/A低于轻度子痫前期组、妊娠高血压组及对照组(P<0.05)。重度子痫前期组Catestatin、Salusin-α低于轻度子痫前期组、妊娠期高血压组及对照组(P<0.05),脑钠尿肽高于轻度子痫前期组、妊娠期高血压组及对照组(P<0.05)。Pearson相关分析显示,Catestatin、Salusin-α与妊娠期高血压疾病患者A峰、AVTI呈负相关(r=-0.642、-0.678;-0.630、-0.655,均P<0.05),与E峰、EVTI、E/A呈正相关(r=0.563、0.651、0.712;0.602、0.637、0.703,P<0.05)。Logistic回归分析显示,收缩压、舒张压、脑钠尿肽、Catestatin、Salusin-α是妊娠期高血压和子痫前期患者左心室舒张功能变化的独立影响因素[OR(95%CI)1.042(1.020~1.065),1.053(1.019~1.089),1.063(1.022~1.105),1.650(1.109~2.457),1.726(1.142~2.611)]。结论血清Catestatin、Salusin-α水平与妊娠期高血压和子痫前期严重程度相关,可能与左心室功能损伤有关。Objective To investigate the relationship between the levels of Catestatin,Salusin-αin serum and the decreased left ventricular function in gestational hypertensives.Methods From May 2016 to August 2018,216 gestational hypertensives were enrolled in this study.The participants were divided into gestational hypertension group(n=84),mild preeclampsia group(n=75)and severe preeclampsia group(n=57).Additionally,a total of 100 normal pregnant women admitted to our hospital during the same period were selected as the control group.The basic data,left ventricular diastolic function and the levels of Catestatin,Salusin-αin serum were compared among the three groups.Pearson correlation analysis and Logistic regression analysis were performed.Results The body mass index(BMI),systolic blood pressure and diastolic blood pressure in severe preeclampsia group were significantly higher than those in mild preeclampsia group,gestational hypertension group or control group(P<0.05).Peak A,A peak velocity time integral(AVTI)of mitral valve forward blood flow were higher,while peak E,E peak velocity time integral(EVTI),E/A were lower in severe preeclampsia group as compared with the other three groups(all P<0.05).The serum Catestatin,Salusin-αwere lower,while brain natriuretic peptide(BNP)was higher in severe preeclampsia group than those in the groups of mild preeclampsia,pregnancy hypertension,and control(all P<0.05).Catestatin and Salusin-αwere negatively correlated with peak A and AVTI(r=-0.642,-0.678;-0.630,-0.655,all P<0.05)and positively correlated with E peak and EVTI,E/A(r=0.563,0.651,0.712;0.602,0.637,0.703,all P<0.05)in gestational hypertension and preeclampsia patients,as indicated by Pearson correlation analysis.Systolic blood pressure,diastolic blood pressure,BNP,Catastatin,and Salusin-αwere the independent factors of left ventricular diastolic function in gestational hypertension and preeclampsia patients[OR(95%CI)1.042(1.020-1.065),1.053(1.019-1.089),1.063(1.022-1.105),1.650(1.109-2.457),1.726(1.142-2.611)],as su
